摘  要:探讨Fas、FasL系统表达、突变与大肠癌转移的关系。方法 :采用RT -PCR和PCR -SSCP方法检测大肠癌原发灶、肝转移灶、癌旁肠粘膜、门静脉血淋巴细胞中Fas、FasL表达、突变。结果 :在 86例大肠癌原发灶、肝转移灶、癌旁肠粘膜、门静脉血淋巴细胞中Fas表达阳性率分别为 67.4 % ,63 .2 % ,4 1.9% ,87.2 %。Fas突变率分别为 50 .0 % ,70 .8% ,2 5.% ,8.0 %。在大肠癌原发灶、肝转移灶中Fas突变率较高 (P <0 .0 1)。3 8例肝转移灶中全部有FasL表达 ,而在原发灶、癌旁肠粘膜、门静脉血淋巴细胞中FasL表达阳性率分别为2 5.6% ,14 .0 % ,50 .0 %。肝转移灶中FasL表达高于其它组织 (P <0 .0 1)。结论 :Fas突变和FasL增强表达是大肠癌细胞逃避免疫监视和杀伤并发生肝转移的机制之一。To study the relationship of expression and mutation of Fas and FasL with hepatic metastasis of colorectal carcinoma. RT-PCR and PCR-SSCP was used to detect expression and mutation of Fas and FasL in primary foci, hepatic metastasis, paracancer mucosa and lymphocytes in portal vein blood. In 86 cases of large intestinal cancer, the positive rates of Fas expression in primary foci, hepatic metastasis, paracancer mucosa and the blood lymphocytes were 67.4%, 63.2%, 41.9%, 87.2% respectively. And the rates of Fas mutations were 50.0%, 70.8%, 25.0%, 8.0% in the different tissues. The rates of Fas mutations in primary foci and hepatic metastasis were higher than that in paracancer mucosa and the blood lymphocytes (P<0.01). The positive expression of FasL was discovered in all 38 hepatic metastasis. But in primary foci, paracancer mucosa and blood lymphocytes, FasL expression rates were 25.6%, 14.0%, 50.0% respectively. The expression of FasL in hepatic metastasis was higher than that in other tissues (P<0.01). But the contrast of FasL mutations in the tissues was no significant difference. Mutation of Fas and expression of FasL in large intestinal cancer cells is one of the mechanisms with which carcinoma cells escape from immune surveillance and being killed and result in hepatic metastasis.
