摘 要:目的研究比较Er,Cr:YSGG激光与传统牙钻均分别预备老年及青年离体磨牙Ⅴ类洞对复合树脂充填微渗漏的影响。方法于2012年5月~9月,在中国医科大学口腔医院,将10颗老年及10颗青年因牙周病或阻生拔出的无龋坏、无充填物、无隐裂的磨牙,按预备方式不同,平均分为四组:第一组,老年高速;第二组,老年激光;第三组,青年高速;第四组青年激光,每组5颗牙。经200次冷热循环染色后,将上述所有样本置于0.5%碱性品红溶液中浸泡24小时后,沿牙体长轴通过充填体正中纵行剖开,在体视显微镜下根据染料渗入等级评分(0~3)及扫描电镜方法观察充填体微渗漏情况。结果牙合壁和龈壁处,染料渗入结果显示,老年组和青年组染料渗入评分差异有统计学意义(P<0.05),老年组大于青年组;激光组和牙钻组差异无统计学意义(P>0.05)。充填体边缘微缝隙宽度的结果显示,4组充填体边缘微缝隙宽度差异存在统计学意义(P<0.05),其中老年牙钻组的边缘微裂隙最大,青年激光组的边缘微裂隙最小。结论在自酸蚀粘结剂的应用条件下,Er,Cr:YSGG激光预备处理Ⅴ类洞不能显著减少光固化复合树脂边缘微渗漏的发生;老年磨牙组与青年磨牙组相比较,无论是Er,Cr:YSGG激光还是牙钻预备光固化复合树脂充填后,微渗漏评分及边缘微缝隙均大于青年磨牙组。Objective The purpose of this study was to compare and study microleakage of Class Ⅴcomposite restorations following high-speed rotary and Er,Cr:YSGG laser preparation old and young molars.Methods From May 2012 to September,in the Stomatological Hospital of China Medical University,caries-free vital ten old and ten young molars freshly extracted for periodontic or-orthodontic reasons were used in this study.Prepare different ways are divided into four groups:The first group old high-speed;The second group old Er,Cr:YSGG laser;The third group young high-speed;The four group young Er,Cr:YSGG laser;each group of 5 teeth.After thermocycling for 500 times.Then all specimens were stained in 0.5% magenta solution for 24 hours.Longitudinal sections were obtained and study with a stereomicroscope for assessment of the microleakage according to degree of dye penetration(scale 0~3) and SEM.Results On either the occlusal walls or gingival walls,Old dye penetration rating is greater than the youth,Er,Cr:YSGG laser and high-speed was found no statistical differences(P0.05).Pack edge microleakage slit width results show that:Four groups pack edge microleakage slit width difference exist statistical significance.Old high-speed group of edge microleakage maximum and youth laser group of edge microleakage minimum.Conclusion Er,Cr:YSGG laser irradiation do not reduce the microleakage of composite resin restorations significant differences using self-etch adhesive system.Whether Er,Cr: YSGG laser or dental drill prepare light cured composite resin after filling old molars group and youth molars compared group,Microleakage seepage score and the edge microleakage crack are greater than youth molars group.
