TfR1/CD71在甲状腺乳头状癌中表达的意义

摘　　要：目的探讨TfR1/CD71在甲状腺乳头状癌(PTC)中的表达和鉴别诊断中的价值。方法复习60例PTC、32例结节性甲状腺肿伴乳头状增生(NGWPH)及18例滤泡性腺瘤(FA)患者的资料,观察HE切片并运用免疫组织化学方法检测TfR1/CD71的表达并与CK19、34βE12、HBME-1、Galectin-3和CD56的表达作比较。结果 TfR1/CD71在PTC组、NGWPH组和FA组的阳性率分别为93.3%、9.4%、11.1%,在甲状腺良恶性病变中的表达有明显差异性(P<0.05),其诊断PTC的敏感度为93.3%,特异度为90.0%,准确度为91.8%。CK19、34βE12、HBME-1、Galectin-3和CD56诊断PTC的敏感度、特异度和准确度分别为98.3%,86.0%,92.7%;71.7%,98.0%,83.6%;85.0%,92.0%,88.2%;90.0%,78.0%,84.5%;96.7%,100.0%,98.2%。结论 TfR1/CD71在PTC的诊断与鉴别诊断中具有一定的价值。Objective To investigate the TfR1/CD71 significance of the value of expression and differential diagnosis in papillary thyroid carcinoma（PTC）.Methods Review was conducted of 60 cases of PTC 32 cases of nodular goiter with papillary hyperplasia（NGWPH） and 18 cases of follicular adenoma（FA） patient information.HE sections and the use of immunohistochemistry were used to detect the expression of TfR1/CD71 and comparision was made with expression of CK19,34βE12,HBME-1,Galectin-3 and CD56.Results TfR1/CD71 positive rates in the PTC group,the NGWPH group and the FA group were 93.3%,9.4%,11.1%,with a significant difference（P0.05） expression in thyroid benign and malignant lesions;the sensitivity,specificity and accuracy for the diagnosis of PTC were 93.3%,90.0%,91.8%.The sensitivities of CK19,34βE12,HBME-1,Galectin-3,and CD56 were 98.3%,71.7%,85.0%,90.0%,96.7%;the specificities was 86.0%,98.0%,92.0%,78.0%,100.0%;Overall accuracies were 92.7%,83.6%,88.2%,84.5%and 98.2%,respectively.Conclusion TfR1/CD71 has a certain value in diagnosis and differential diagnosis of PTC.
