摘  要:目的观察蛛网膜下腔出血(SAH)后大脑中动脉超微结构的变化及内皮型一氧化氮合酶(eNOS)基因对它的影响。方法采用枕大池二次注血法制备兔SAH模型。24只兔随机分成对照组、SAH组、携带eNOS基因重组腺病毒(AdeNOS)组。AdeNOS组造模前用微量泵经颈内动脉将AdeNOS缓慢泵入转染大脑中动脉内皮细胞。对照组枕大池内先后两次注射生理盐水。实验动物于首次注血后7d进行灌注固定,留取大脑中动脉标本,在电镜下观察其超微结构的改变。结果电镜下,对照组血管壁超微结构无变化,SAH组血管内皮层结构以及中层细胞结构发生严重变性;AdeNOS组组织变性程度明显轻于SAH组。结论 SAH可引起血管内膜和中层超微结构发生明显的变性,eNOS基因可明显减小脑动脉壁内膜和中层超微结构的损伤,提示其可用于SAH后脑血管痉挛的预防。Objective To study the effect of endothelial nitric oxide synthase (eNOS) gene on the ultrastructural changes of the middle cerebral artery (MCA) after the subarachnoid hemorrhage (SAH) in rabbits. Methods Twenty-four rabbits were randomly divided into three groups, i.e. control group (n=8), SAH group (n=8) and eNOS group (n=8). SAH models were made by two injections of 2.5 ml fresh and non-heparinized arterial blood into the citerna magna of the rabbits in SAH and eNOS groups. The same valome physiological saline was injected into the citerna magna of rabbits in the control group. The recombinant adenovirus (8× 1010 pfu/ml) with eNOS gene was infused into the internal carotid artery of the rabbits before SAH in eNOS group. All the rabbits were killed 7 days after SAH, and MCA were isolated for ultrastruetural observation with a transmission electron microscope. Results The ultrastructures of MCA were unchanged in the control group. Swelling, degeneration (even necrosis), and vacuolization were seen in most endothelial cells of MCA, of which, cytoplasmic electron density declined in SAH group. The disruption of tight junctions also were observed and some intima shed into the lumen in SAH group, in which the irregularity and swelling could be seen in the smooth muscle cells with reduced organelles. However, the changes in MCA uhrastructures were significantly lighter in eNOS group than those in SAH group. The tight junctions become more complete than those in SAH group and no intima shed into the lumen in eNOS group, where though mildly swelling could be seen in some mitochondria, but their morphologies were normal by and large. The vacuolization of MCA endothelial cell were less in eNOS group than that in SAH group. No obvious distortion of elastic membrane was detected in the MCA walls, and the morphology of MCA smooth muscle cells were slightly changed in eNOS group compared to SAH group.
