护士、医生与临床药师合作处理静脉输液相关药患纠纷的实践

摘　　要：目的使药患纠纷得到及时、妥善的处理。方法设立药患纠纷处理专门小组,完善制度;快速反应,通知医生、临床药师协同处置;综合分析判断,各司其职分类处理;讨论总结,持续改进。结果 27例药患纠纷均得到了圆满解决,有效化解了矛盾。通过和药师密切合作处理药患纠纷,及时整改,2008～2012年的药患纠纷分别为15例、5例、4例、2例、1例,药患纠纷逐年减少。结论护士、医生和临床药师合作处理药患纠纷,有利于药患纠纷的化解和处理,有利于药物不良反应的监测和上报。Objective To solve the medicine disputes timely and appropriately. Method A medicine disputes group was set up. Accordingly,the regulations and responsibilities were formulated. The physicians and clinical pharmacists would cooperate with nurses to solve the medicine disputes after comprehensive analysis and judgment. Then,they summarized the process together for continuous improvement. Results Twenty-seven cases of medicine disputes were well solved without economic compensation. Conclusion Solving medicine disputed cooperated by nurses and clinical pharmacists can better solve the disputes and monitor the report adverse drug reactions,reduce the risk of intravenous infusion, improve the quality of medication administration,reduce and avoid medicine disputes and ensure patient safety.
