抗gp210、Sp100抗体对原发性胆汁性肝硬化的诊断意义

摘　　要：目的探讨抗gp210和Sp100抗体对原发性胆汁性肝硬化的诊断意义。方法应用免疫印迹法检测129例PBC以及1 206例其他患者的抗gp210、Sp100抗体及抗线粒体抗体M2亚型(AMA-M2),分别计算抗gp210、Sp100抗体及AMA-M2对PBC诊断的敏感性、特异性、阳性预测值、阴性预测值。结果 129例PBC患者中,抗gp210、Sp100抗体及AMA-M2阳性者分别为49、34和84例,抗gp210、Sp100抗体及AMA-M2诊断PBC的敏感性分别为:38.0%、26.4%和65.1%,特异性分别为:99.4%、99.0%和97.7%。阳性预测值分别为87.5%、73.9%和75.0%;阴性预测值分别为93.7%、92.6%和96.3%。AMA-M2阴性的PBC共45例,其中26例(57.8%)gp210抗体阳性,27例(60.0%)Sp100抗体阳性。结论抗gp210抗体以及抗Sp100抗体对PBC有高度的特异性,与AMA-M2联合检测可以提高对PBC的检出率并增加对PBC诊断的特异性,抗gp210及Sp100抗体对PBC的诊断尤其是AMA-M2阴性患者具有重要意义。[Objective] To examine the diagnostic value of gp210 and spl00 antibodies in patients with primary biliary cirrhosis （PBC）. [ Methods ] The gp210, spl00 antibodies and antimitochondrial antibodies M2 subtype （A- MA-M2） were detected by immunoblotting in 129 patients with PBC and 1 206 patients with other diseases respectively, and sensitivity, specificity, positive predictive value （PPV） and negative predictive value （NPV） of the three antibodies were compared. [Results] In 129 PBC patients, gp210, spl00 antibodies and AMA-M2 were positive in 49, 34, 84 cases respectively. Sensitivity of the three antibodies were 38.0%, 26.4%, 65.1% respectively, and speci- ficity were 99.4%, 99.0%, 97.7% respectively, PPV were 87.5%, 73.9%, 75.0% respectively, NPV were 93.7%, 92.6%, 96.3% respectively. In all 45 PBC patients of AMA-M2 negative, gp210 antibodies were positive in 26 cases （57.8%）, and spl00 antibodies were positive in 27 cases （60.0%）. [Conclusion] The specificity of gp210 and spl00 antibodies were higher for the diagnosis of PBC, and combination of the two antibodies and AMA-M2 can increase the detection rate and diagnostic specificity of PBC. The detection of gp210 and sp100 antibodies has great significance to diagnose PBC, especially in the AMA-N2 negative patients with PBC.
