摘 要:目的观察血管内皮生长因子(VEGF)、表皮生长因子受体(EGFR)在进行期寻常型银屑病皮损及非皮损中的表达及其与PASI指数的关系,探讨银屑病发生发展中表皮增殖和血管生成的关系。方法以30例寻常型进行期银屑病患者为研究对象,记录患者的银屑病皮损面积和严重度(PASI)积分,采用免疫组化技术(PV-9000两步法)检测30例进行期寻常型银屑病皮损及其邻近皮肤、15例正常皮肤中VEGF、EGFR的表达情况。结果①皮损组VEGF表达总阳性率为100%,明显高于邻近组和正常对照组,具有显著性差异;EGFR表达总阳性率为100%,与邻近组相比无显著性差异,明显高于正常对照组,具有显著性差异。②皮损组VEGF阳性染色评分与PASI积分无相关性(rs=0.2821,P>0.05),EGFR阳性染色评分与PASI积分呈正相关(rs=0.4529,P<0.05)。③VEGF阳性染色评分与EGFR阳性染色评分无相关性(r=0.2247,P>0.05)。结论①VEGF、EGFR在进行期寻常型银屑病患者皮损中异常高表达。②VEGF与PASI积分之间无相关性,提示VEGF与银屑病的病情活动性有关,而与疾病的严重程度无关。EGFR与PASI呈正相关,提示EGFR与银屑病的严重程度相关,而与疾病的活动性无关。③VEGF和EGFR无相关性提示两者在银屑病的发病机制中无直接的协同作用。Objective To observe the expression of vascular endothelial growth factor (VEGF) and epidermal growth factor receptor (EGFR) in lesional and non-lesional skin of patients with psoriasis vulgaris, and the relationship with the PASI (Psoriasis Areas and Se- verity Index), and further investigate the correlation between epidermal proliferation and angiogenesis in the pathogenesis of psoriasis. Methods 30 patients who were in progressive stage of psoriasis vulgaris were collected in this study and their PASI score was recorded. The expression of vascular endothelial growth factor(VEGF) and epidermal growth factor receptor(EGFR) in lesional or non-lesional skin of 30 patients with psoriasis vulgaris, and 15 healthy controls were detected by two-step immunohistochemical staining method. Results The expression rates of VEGF in lesional skin was 100% , which was significantly higher than that in non-lesional and normal skin. The positive rate of EGFR expression was 100% in lesional and non-lesional skin, which was significantly higher than that in normal skin. There was no correlation between the score of VEGF positive stain and PASI (P〉0.05)in lesional skin, but the score of EGFR positive stain were positively correlated with the PASI( rs = 0. 4529 ,P〈0.05 ). There was no correlation between the score of VEGF positive stain and EGFR( r= 0. 2247 ,P〉0. 05). Conclusion The expression of VEGF and EGFR was significantly up-regulated in active stage of pso- riasis vulgaris. There was no correlation between VEGF and PASI, which suggested that VEGF was related to the activity of psoriasis, but not to the severity of disease. There was significant correlation between EGFR and the score of PASI, which indicated that EGFR was re- lated to the severity of psoriasis, but not to the activity of disease. There was no correlation between VEGF and EGFR, which suggested that they was no directly synergistic effect in the pathogenesis of psoriasis.
