香菇多糖在结直肠癌患者术后辅助化疗中的应用

摘　　要：目的:探讨FOLFOX4方案单独化疗与香菇多糖注射液联合FOLFOX4化疗治疗结直肠癌患者的疗效与不良反应。方法:78例结直肠癌患者,随机分为对照组37例和治疗组41例,治疗组给予香菇多糖注射液联合FOLFOX4方案化疗,对照组给予FOLFOX4方案单独化疗。监测治疗前后中医症候疗效、生活质量评分、免疫功能变化及不良反应,并评价疗效。结果:治疗组和对照组的有效率和临床获益率比较,差异无统计学意义(P>0.05)。治疗后的中医征候疗效及KPS评分治疗组明显高于对照组(P<0.05)。治疗组治疗后CD3+、CD4+、CD4+/CD8+及NK细胞比例明显增高,而CD8+百分率明显降低,差异具有统计学意义(P<0.05)。治疗组胃肠道反应、骨髓抑制、肝功能损害及周围神经毒性等不良事件的发生率低于对照组,差异有统计学意义。结论:香菇多糖注射液联合FOLFOX4方案治疗结直肠癌患者,可以提高化疗疗效,改善患者生活质量,提高机体免疫功能,减轻化疗所致的不良反应,明显起到辅助、增效作用。Objective：To compare the efficacy and toxicity between chemotherapy of FOLFOXd and Lentinan injection combined with FOLFOX4 chemotherapy to treat patients with colorectal cancer. Methods ： Seventy - eight patients were randomly divided into two groups, control group（37 cases） and treatment group（41 cases）. The control group was treated with FOLFOX4 only, and treatment group was treated by combining Lentinan injection and FOLFOX4. The therapeu- tic efficacy was evaluated and KPS score and side effects were observed. Syndrome scores of traditional Chinese medicine and immune status were also determined. Results：There was no difference in the objective response rate between the treatment group and the control group （ P 〉 0.05 ）. KPS score and syndrome scores of traditional Chinese medicine of the treatment group after the treatment was significantly higher than the that of the control group（ P 〈 0.05 ）. The immunological function detection showed that CD3^＋ , CD4^＋ , CD4^＋/CD8^＋ , NK cell increased obviously post- therapy than pretherapy in the treatment group （ P 〈 0.05 ） and CDs decreased after treatment （ P 〈 0.05 ）. The incidence of gastrointestinal reac- tion, blood toxicity, liver dysfunction and peripheral nerve toxicity in treatment group was less than those in the control group, the differences were statistically significant （ P 〈 0.05 ）. Conclusion ： The study suggests that Lentinan injection combined with FOLFOX4 chemotherapy in treatment of colorectal cancer is more effective and has fewer side effects. It also can improve the life quality and immunological function of the patients and obviously plays a supporting and synergistic effect.
