摘 要:目的观察下肢康复机器人对缺血性脑卒中偏瘫患者下肢功能的影响。方法选取河北联合大学神经内科住院治疗的缺血性脑卒中恢复期的偏瘫患者40例,分为对照组(20例)和Lokomat组(20例)。两组患者均采用采用常规康复疗法,Lokomat组还另外采用下肢康复机器人的运动训练方法,两组的患者总疗程为2个月。均在治疗前、治疗5周后及治疗10周后采用Fugl-Meyer下肢运动评分、关节活动度作为评估指标进行评价比较。关节活动度测定髋膝关节的最大活动范围、下肢Fugl-Meyer运动量表测定用以评定下肢运动功能(FM—LE)共17个项目,最高分为34分。结果两组患者偏瘫侧Fugl-Meyer下肢运动评分、关节活动度均较治疗前明显改善(P﹤0.05),且以Lokomat组的改善幅度较显著,与对照组比较,差异有统计学意义(P﹤0.05);两组患者治疗前偏瘫侧肢体Fugl-Meyer下肢运动评分、关节活动度无明显的变化(P﹥0.05)。结论 Lokomat下肢康复机器人能明显改善恢复期偏瘫患者的下肢功能,具有显著促进作用。OBJECTIVE To investigate the effects of Lokomat lower gait training rehabilitation robot on lower limb function in hemiplegic stroke patients. METHODS 40 hemiplegic stroke patients were divided into Lokomat group and a control group with 20 cases in each group. The lokomat group received Lokomat rehabilitation therapy combined with routine rehabilitation training, while the control group only received routine rehabilitation for 2 months respectively. All patients were evaluated by Fugl-Meyer motor score and range of motion before and 5 and 10 weeks after treatment. RESULTS Before treatment there was no significant difference in the Fugl-Meyer motor score and range of motion between the two groups ( P 〉 0.05 ). After treatment all indexes were improved greatly (P 〈 0.05 ), and the improvement in lokomat treatment group was significantly better than that in the control group (P 〈 0.05). CONSLUSION The Lokomat gait training rehabilitation robot can improve lower limb function in hemiplegic stroke patients.
