摘　　要：目的:分析上海市二级医院医疗纠纷事件的分布、原因及解决途径,探索医疗纠纷发生规律,为政府和医疗机构制定科学的干预策略和措施提供依据。方法:以上海市松江区某二级医院为研究对象,分析2008～2011年医疗纠纷事件的时间、科室、人群分布特征,比较不同原因及解决途径在科室及不同年龄人群的分布差异。结果:医疗纠纷无明显的时间集中趋势,纠纷发生前5位的科室分别为普外科、骨科、妇产科、GICU及放射科,占总发生数的56%。纠纷涉及的患者平均年龄为50岁左右,医疗纠纷发生的原因主要为医护技术原因,约为52%。解决途径主要是双方协商解决,占90%左右。发生原因和解决途径在科室和不同年龄组分布差异无统计学意义(P>0.05)。结论:高年龄组、外科科室是上海市松江区二级医院医疗纠纷的危险人群和高发科室,医护技术原因是主要发生原因,政府和二级医疗机构应加强对其干预和防范。Objective To analyze the distribution, causes and solutions to medical disputes in the secondary hospitals in Shanghai, explore the law of medical disputes, and provide references for the government and hospitals to put forward scientific countermeasures. Methods One secondary hospital was selected from Songjiang District. The characteristics of distribution of medi- cal disputes events from 2008 to 2011 were comparatively analyzed from aspects of time, departments and population. Results There had no significant difference of time distribution for medical dispute events. The top five departments were surgical depart- ment, orthopedics, obstetrics and gynecology, GICU and radiology, which accounted for 56% of the total occurrence. The average age of patients involved in medical dispute was 50 years old. And the main reason of medical disputes was medical technology, which accounted for 52%. Consultation was the main solution to medical disputes, accounting for about 90%. There had no signif- icant differences of causes and solutions among different departments and ages （ P 〉 0.05 ）. Conclusion Medical dispute events were mainly occurred in older group and surgical departments in the secondary hospitals in Songjiang District of Shanghai. And its main cause was medical technology,which needs to be intervened by the government and medical institutions.
