摘　　要：目的为解决三维标测指导下房颤导管消融手术医生缺乏训练手段的问题,根据常用的三维标测CARTO房颤手术系统原理,研制一种基于人在回路感知的房颤导管消融训练器。方法分析人在回路的CARTO系统手术过程,提出训练器系统总体结构,并给出实现该系统的关键技术,包括:与实际手术用消融导管手感一致的训练用消融导管实现方法、基于实际病人数据的虚拟病人心脏几何模型和心电数据仿真方法、基于质点弹簧模型的虚拟消融导管仿真方法和基于OBB(oriented bounding box,方向包围盒)包围盒碰撞检测算法的两层虚拟手术交互操作模型。系统集成后邀请专家试用并对其评价。结果该训练器与实际手术系统的操作方法、硬件操作界面和视觉信息一致,该系统交互操作实时、灵活。结论该训练器能完成人在回路的房颤手术过程交互仿真,并可用于医生的手术训练和术前手术预演。Objective The cure of Atrial fibrillation （AF） by ablation catheter is a highly demanding technique with high surgical risks, but the means of actual surgical practice for interns are rather insufficient. Based on the principle of current CARTO AF operation system, the simulator of AF surgical operation for training interns is proposed and described. Methods The surgical procedure by CARTO was analyzed, the system architecture of simulator was put forward, and the key technologies to implement the system was given including an ablation catheter for training purpose which was the same to the one for surgery in operation feeling, simulation of the geometric and electrocardiogram of the virtual heart, virtual ablation catheter based on mass-spring model and the two-tiers model structure of virtual heart and catheter in operation simulation based on OBB collision detec- tion. Experts of AF surgery were invited to evaluate the system through trail usage. Results The method of op- eration, hardware interface and visual information interface of the training simulator were consistent with the actual surgical system. The simulator was real time and flexible when operating. Conclusion The system can interactively simulate the surgical process of AF with human in the loop. It can be used for catheter ablation surgical training for interns and preoperative surgical rehearsal for skilled doctors to improve the success rate of surgery.
