摘 要:目的通过肠瘘病人的营养治疗为例,探讨以问题为基础学习(Problem-based Learning,PBL)教学方法在临床营养学理论教学中的应用。方法以第四军医大学2008级五年制临床医学专业78人,2008级五年制空军临床医学专业106人,2009级四年制护理专业50人为研究对象,通过问卷调查和比较临床营养学各章节期末考试成绩,分析两种教学方法的效果。结果采用PBL教学方法取得了较好的教学效果,学生的考试成绩完全支持这一结论。其中外科疾病的营养治疗优秀率达38.9%,远高于其他各章节;而<5分学生比率仅7.7%。结论PBL教学方法适合临床营养学教学且效果优于LBL教学方法,PBL教学有利于达到教学目标,能更好地实现学生综合能力的培养。但是,目前在医学高等教育中有关PBL教学改革的问卷调查结果过于乐观,其中可能存在偏倚,在今后的研究中值得注意。Objective The nutritional therapy for patients with intestinal fistula patients, to discuss the problem based learning (PBL) teaching method in clinical nutrition theory teaching application. Meth- ods The students of 2008 grade in the Fourth Military Medical University majored in clinical medicine for five years in 78, 2008 grade of five year clinical medicine professional air force 106, 2009 grade in four year nursing professional 50 as the research object, through a questionnaire survey and compari- son of clinical nutrition chapters scores in the final examination, analysis of effectiveness of two teach- ing methods. Results Using PBL teaching methods have achieved good results in teaching, students' examination results fully support this conclusion. Which surgical disease nutritional therapy of excel- lent rate of 38. 9~, far higher than other sections; and ~ 5student ratio is only 7. 7~. Conclusions PBL teaching method suitable for clinical nutrition teaching effect is better than that of LBL and PBL teaching method, teaching helps to achieve the goal of teaching, can better achieve the training of students' comprehensive ability. However, at present in higher medical education teaching reform a-bout PBL questionnaire results too optimistic, there may be bias, in future studies of note.
