胃血管球瘤43例诊治分析

摘　　要：目的:报道和总结胃血管球瘤诊疗经验。方法:收集近10年共43例胃血管球瘤(本院2例,文献报道41例),分析及总结该疾病的主要临床表现、辅助检查、病理学诊断以及治疗经过和疗效。结果:胃血管球瘤多为良性肿瘤,常见症状为腹痛。少数患者以消化道出血为首发症状,且多为恶性胃血管球瘤。其形态学特点常表现为片状肿瘤组织,肿瘤细胞多为大小均匀的圆形细胞,胞浆透亮、较少,围绕于毛细血管周围。免疫组化标记:SMA、vimentin、actin均为(+),CD34多为(-),CK、CD117、S-100、CD99、desmin、Syn均为(-)。结论:胃血管球瘤是较为罕见的胃肠道肿瘤,是由与平滑肌细胞非常相似的血管球细胞构成的间叶性肿瘤。血管球瘤大多为良性,恶性者罕见。CT和内镜是当前较常用的诊断方法,确诊必须依靠病理学检查和免疫组化检测,目前最有效的治疗方法是手术切除。Objective：To summarize the diagnosis and treatment of gastric glomus tumor. Methods： Data of 43 cases of gastric glomus tumor were collected in which 2 cases treated in our hospital and 41 cases reviewed from literature reported in recent 10 years. Clinical features, assistant examinations, pathological evidence and course of treatment for final diagnosis, treatment were analyzed. Results： The tumor are mostly benign, and abdominal pain is the most common symptom. Few patients come to hospital with complains of trointestinal bleeding as the first symptom. Tumors often happen in malignant gastric glomus tumors. Histologically, the tumor cells were uniform, round or polygona, always with clear or eosinophilic cytoplasm, nuclei were round, the tumor cells usually had a solid pattem or arranged around blood vessels. Immunohistochemically, all tumors were positive for vimentin, SMA, actin and little CD34, while other markers including CK, CD117, S - 100, desmin, Syn, CD99 were negative. Conclusion： Gastric glomus tumors are rare tumors of the gastrointestinal tract,which is very similar to the smooth muscle cells originated from mesenchymal tumors. Glomus tumor is mostly benign. CT and endoscopy is more commonly used to make a diagnosis. A definite diagnosis must rely on histopathology and immunohistochemical detection, the most effective treatment is surgical removal.
