眼外肌本体感受器切除联合眼外肌缩短术治疗眼球震颤疗效分析

摘　　要：目的研究眼外肌本体感受器切除联合眼外肌缩短术在治疗无中间带型先天性眼球震颤的疗效。方法对42例无中间带型的先天性眼球震颤患者施行眼外肌本体感受器切除联合眼外肌缩短术，通过比较患者术前术后视力、对比敏感度双眼视功能等指标，以及术后患者主观视物的清晰度改善情况，综合评价该手术在治疗无中间带型先天性眼球震颤患者中的安全性及其效果。结果共有39例患者（92．8％）对手术效果满意，术后6个月时视力提高≥2行者有12例（28．6％），但术后所有患者双眼视功能及对比敏感度在各个空间频率上均有提高。结论眼外肌本体感受器切除联合眼外肌缩短术在治疗无中间带型的先天性眼球震颤方面是安全有效的。对比敏感度检查可用于评价眼球震颤手术的效果。Objective To prove the effectiveness of extraocular muscles proprioceptor tenotomy combined with extraocular muscle resection in treatment of infantile nystagmus syndrome （INS） patients without nullpoint. Methods Forty-two patients who suffered from INS were involved. All the patients received extraocular muscles proprioceptor tenotomy combined with extraocular muscle resection. Horizontal and vertical eye movements were recorded pre- and post-surgically using high-speed digital video. The nystagmus amplitudes and frequencies, contrast sensitivity function and stereopsis were measured pre- and post-surgery. Results The patients with extraocular muscles proprioceptor te- notomy combined with extraocular muscle resection gained obvious improvement in contrast sensitivi- ty and binocular vision. Conclusions Extraocular muscles proprioceptor tenotomy combined with extraocular muscle resection are a safe method in treatment of the INS patients without nullpoint. The contrast sensitivity function can be used to evaluate the effects of nystagmus surgeries.
