摘 要:目的探讨乳腺浸润性导管癌中哺乳动物雷帕霉素靶蛋白(mammalian target of rapamycin,mTOR)与趋化因子受体7(chemokine receptor 7,CCR7)的表达以及与乳腺癌侵袭、转移之间的关系。方法采用免疫组化MaxVision法检测105例乳腺浸润性导管癌、55例转移淋巴结及40例正常乳腺组织中mTOR和CCR7的表达,并分析mTOR和CCR7的表达与乳腺癌侵袭和淋巴结转移的关系以及二者之间的相关性。结果 mTOR在乳腺浸润性导管癌、转移淋巴结及正常乳腺组织中的阳性率分别为68%(71/105)、93%(51/55)、25%(10/40),差异有统计学意义(P<0.05);CCR7在乳腺浸润性导管癌、转移淋巴结及正常乳腺组织中的阳性率分别为74%(78/105)、94%(52/55)、28%(11/40),差异有统计学意义(P<0.05)。在乳腺癌中,mTOR和CCR7的表达呈正相关(rs=0.203,P<0.05)。mTOR和CCR7的高表达均与淋巴结转移有关(P<0.05)。结论mTOR和CCR7的异常高表达可能与乳腺癌的侵袭、转移有关,且检测二者的表达可作为判断乳腺癌转移及预后的预测指标。Purpose To detect the expression of roTOR and CCR7 protein in breast invasive ductal carcinoma, and to analyze the rela- tionship of mTOR and CCR7 protein with the invasion and metastasis of carcinoma of the breast. Methods Immunohistochemistry was used to detect the mTOR and CCR7 protein expression levels in 105 cases of breast invasive ductal carcinoma, 55 cases of metastatic lymph node and 40 cases of normal breast tissue. At the same time, to analyze the relationship of mTOR and CCR7 protein with the in- vasion and metastasis of carcinoma of the breast, and to investigate the correlation between mTOR and CCR7. Results The mTOR protein positive rates in breast invasive ductal carcinoma tissue, metastatic lymph nodes and cancer adjacent tissue were 68% (71/ 105 ), 93 % ( 51/55 ) and 25 % ( 10/40 ), respectively, and the differences were statistically significant ( P 〈 0. 05 ). The CCR7 protein positive rates in breast invasive ductal carcinoma tissue, metastatic lymph nodes and cancer adjacent tissue were 74% (78/105), 94% (52/55) and 28 % (11/40) , with significant difference (P 〈 0. 05 ). And the protein expression level of roTOR was positively correla- ted with CCR7 in breast invasive ductal carcinoma tissue (r~ = 0. 203, P 〈 0. 05 ). The overexpression of mTOR and CCR7 were corre- lated with lymph node metastasis ( P 〈 0. 05 ). Conclusions Abnormal expression of roTOR and CCR7 may be correlated with the oc- currence, development, invasion and metastasis of breast cancer, and detection of mTOR and CCR7 protein expression levels could be used to predict the metastasis and the prognosis of breast carcinoma.
