意识障碍患者脑脊液乳酸水平与预后的关系

摘　　要：目的探讨意识障碍患者脑脊液乳酸(CFS-LA)水平在预后评判中的价值。方法将2011年11月—2012年12月该院收治的40例意识障碍患者,意识水平分为嗜睡、昏睡、浅昏迷、中昏迷、深昏迷5个意识水平,按Glasgow昏迷量表(GCS)评分,腰穿收集脑脊液做乳酸测定,患者预后分为生存组与死亡组,分析意识水平、GCS评分与CFS-LA水平的相关性,比较生存组与死亡组脑脊液水平的差异。结果经Spearman相关分析,CFS-LA与意识水平呈正相关(r=0.674,P=0.001),CFS-LA与GCS评分呈负相关(r=-0.792,P=0.000),生存组CFS-LA(3.51±0.99),死亡组CFS-LA(4.22±1.59),经χ2检验,两组差异有统计学意义(χ2=11.34,P<0.05)。结论意识障碍患者CFS-LA水平与意识障碍程度相关,可作为预后判断的指标。Objective To investigate the dynamic changes in lactate levels of cerebrospinal fluid（CSF-LA） in patients with traumatic brain injury and its value in prognosis evaluation.Methods According to Glasgow coma scale（GCS）,40 patients with traumatic brain injury in our department from Nov.2011 to Dec.2012 were divided in sleepiness,deep slumber,light coma,in coma and deep coma 5 level.Get the cerebrospinal fluid by lumbar puncture to detect the concentration of lactate.The prognosis were divided in survival group and death group,the relationship of consciousness level,GCS scour and lactate levels were analyzed.Compare the difference of cerebrospinal fluid between the 2 group.Results Analysis by Spearman CFS-LA and consciousness level are positively correlated（r=0.674,P=0.001）,CFS-LA and the score of GCS are negative correlation（r=0.792,P=0.000）.The survive group CFS-LA（3.51±0.99）,death group CFS-LA（4.22±1.59）.By χ2 test,the difference between the two group has a statistically significant.Conclusion The level of CFS-LA and the degree of disturbance of consciousness is related.The level of CFS-LA can be a indicator of prognostic.
