摘  要:目的:探讨江苏省淮安地区居民饮食、生活习惯和肿瘤家族史等因素与食管癌发生的关系。方法:采用以人群为基础的1:1配对病例对照研究的方法,对905例食管癌患者和905例对照者进行问卷调查,采用单因素和多因素条件Logistic回归对所获资料进行分析。结果:多因素条件logistic回归分析结果表明,进食速度过快[比值比(odd sratio,OR)=1.59,95%可信区间(conidence interval,CI):1.21~2.09]、进食食品过咸(OR=1.27,95%CI:1.02~1.57)、粮食有霉变可能(OR=2.45,95%CI:1.46~4.11)、吸烟(OR=1.80,95%CI:1.27~2.54)、性格忧郁(OR=1.53,95%CI:1.16~2.03)、曾患过食管炎(OR=3.91,95%CI:1.08~14.11)、有肿瘤家族史(OR=2.05,95%CI:1.42~2.95)和食用咸鱼咸肉较多(OR=1.50,95%CI:1.01~2.22)可能是食管癌的危险因素;近5年人均年收入高(OR=0.77,95%CI:0.71~0.84)、体质指数较高(OR=0.33,95%CI:0.24~0.44)和食用适量红肉(OR=0.70,95%CI:0.52~0.94)可能是食管癌的保护因素。结论:饮食习惯、生活方式、经济状态、既往食管病变和肿瘤家族史等可能是淮安地区食管癌发病的主要影响因素。Objective: To investigate the risk factors of diet,lifestyle and family history of cancer for esophageal cancer in Huai'an District of Jiangsu Province,China.Methods: A population-based case- control study was conducted and the information was collected using a questionnaire in 905 patients with esophageal cancer and 905 matched controls (1:1 matched).The potential risk factors related to esophageal cancer were analyzed by univariate analysis and conditional logistic regression.Results: In the conditional logistic regression analysis,eating quickly OR (odds ratio) = 1.59,95% CI (confidence interval): 1.21-2.09,sodium-heavy diet (OR = 1.27,95% CI: 1.02-1.57),eating moldy or spoiled food (OR = 2.45,95% CI: 1.46-4.11),cigarette smoking (OR = 1.80,95% CI: 1.27-2.54),depression (OR = 1.53,95% CI: 1.16-2.03),a history of oesophagitis (OR = 3.91,95% CI : 1.08-14.11),a family history of cancer (OR = 2.05,95% CI : 1.42-2.95),and eating more salted fish and bacon (OR = 1.50,95% CI : 1.01-2.22) significantly increased the risk of esophageal cancer; but higher annual net income per capita duringprevious five years (OR = 0.77,95% CI : 0.71-0.84),higher BMI (body mass index) (OR = 0.33,95% CI : 0.24-0.44),and eating a moderate amount of red meat (OR = 0.70,95% CI : 0.52-0.94) were protective factors.Conclusion: Eating habits,life-style,economic condition,history of esophageal illness and a family history of cancer may be the major risk factors for esophageal cancer in Huai'an District.
