精神分裂症患者认知功能状况的多维度探讨

摘　　要：目的从多个维度探讨精神分裂症患者的认知功能缺陷。方法采用中文版精神分裂症认知功能成套测验(MCCB)作为评价工具,测评65例精神分裂症患者和65例正常对照者,对测评结果进行分析。结果①病例组MCCB评分明显低于对照组,MCCB总分(t=5.21,P<0.001),符号编码(t=5.80,P<0.001),持续操作(t=4.40,P<0.001),言语记忆(t=5.13,P<0.001),视觉记忆(t=3.98,P<0.001),空间广度(t=3.05,P<0.05),迷宫(t=2.63,P<0.05),字母数字序列(t=2.26,P<0.05),连线测验(t=2.23,P<0.05)均有显著差异。涉及神经认知的6个纬度,包括处理速度、注意性/警觉性、工作记忆、言语学习和记忆、视觉学习和记忆、推理与问题解决能力;②社会认知方面情绪管理评分(t=0.88,P>0.05),两组比较无显著差异。结论精神分裂症患者神经认知方面存在多维度认知功能损害。Objective To explore the characteristics of cognitive function deficits in patients with schizophrenia. Methods A to- tal of 65 patients with schizophrenia and 65 healthy controls were assessed with Matrics Consensus Cogfiitive Battery（MCCB）. Re- sults ①Patients with schizopnreain were significantly lower than controls in MCCB total score （t=3.21 ,P〈0. 001） symbol cod- ing （t = 5.80,P〈0. 001 ）,continuous performance test-identical pairs （t= 4.40,P〈0. 001）,hopkins verbal learning test-revised （t 5. 13, P〈 0.001 ）, brief visuospatial memory test-revised （t = 3.98, P〈 0.001 ）, spacial span （t = 3.05, P〈 0.05）, neuro psychologi- cal assessment Battery-Mazes（t= 2.63 ,P〈0. 05） ,letter number sequence（t= 2.26 ,P〈0.05） and trial making A （t=-2.23 ,P〈 0.05）. The cognitive function deficits in patients with schizophrenia were shown in 6 domains of neuro-cognition.②There was no significant difference in managing emotion of social cognition between two groups（t=0.88,P〉0. 05）. Conclusion Patients with schizophrenia show various cognitive function impairments in neuro-cognition.
