摘 要:目的分析BCL-2、BCL-6及p53异常在弥漫大B细胞淋巴瘤(DLBCL)的预后价值。方法采用荧光原位杂交法(FISH)对80例DLBCL患者的淋巴结石蜡组织进行BCL-2、BCL-6及p53基因的检测,Cox比例风险回归模型筛选预后因素,卡普兰曲线估计生存率和创建曲线。结果 BCL-2阳性患者相对于阴性患者赋予的相对风险是6.237(P=0.041);p53缺失患者相对于非缺失患者相比赋予的相对风险是10.885(P=0.024);BCL-6基因异常的患者相对于非异常患者赋予相对风险是7.125(P=0.052)。p53及BCL-2基因异常的患者总体生存时间明显低于正常患者(P<0.001vs P=0.012)。而BCL-6基因异常的患者总体生存时间和正常患者比较无明显差异(P=0.426)。结论 DLBCL患者的BCL-2及p53基因异常可作为独立的预后因素,在疾病诊断初期即对DLBCL的BCL-2及p53表达进行检测有利于对其预后进行准确判断。Objective To investigate the prognostic significance of BCL2,BCL6 and P53 gene abnormalities in patients with diffuse large B cell lymphoma (DLBCL).Methods BCL2,BCL6,or P53 genes were detected by means of fluorescence in situ hybridization (FISH) in the paraffin tissues of lymph nodes of 80 patients with DLBCL. Cox proportional hazard regression model was used to screen prognostic factors and Kaplan curve was used to evaluate the survival rate and create the curves, Resulls Patients with positive BCL2 gene had a relative risk of 6.237 ( P = 0.041) comparing to those with negative BCL2 gene.Patients with P53 gene deletion showed a relative risk of 10.885 ( P =0.024) comparing to those patients with P53 gene normalities.And the patients with BCL6 gene abnormalities showed a relative risk of 7.125 ( P = 0.052) comparing to patients with BCL6 gene normalities.The overall survival time of patients with P53 gene and BCL2 gene abnormalities was significantly lower than that of patients with gene normalities( P=0.001vs P =0.012),and there was no significant difference in the overall survival time of patients with BCL6 gene abnormalities and those without BCL6 gene abnormalities( P =0.426).Conclusion BCL2 and P53 gene abnormalities in DLBCL patients can serve as an independent prognostic factor. Detection of BCL2 and P53 gene of patients with DLBC1. favour the prognosis judge in early stage of diagnosis.
