摘 要:【目的】探讨BCP、PreC和C区基因突变率及突变热点与HBV母婴传播免疫失败的关系。【方法】选取血清HBsAg阳性且HBV DNA定量>1000 U/mL的孕妇,以所分娩的新生儿是否发生免疫失败和孕妇血清HBV DNA定量水平将孕妇分为免疫失败组(19例)和对照1组(免疫成功且HBV-DNA定量≥1.0×107U/mL,48例)、对照2组(免疫成功且HBV-DNA定量<1.0×107U/mL,38例)。对3组孕妇血中HBV进行BCP/PreC/C区基因PCR扩增和测序,比较3组的突变率和突变位点的不同。【结果】免疫失败组与对照1组间HBV DNA定量相比,差异无统计学意义(P>0.05),但均大于对照2组(P<0.05);BCP、PreC、C区基因突变率,免疫失败组与对照1组间相比,差异无统计学意义(P>0.05),但均低于对照2组(P<0.05)。突变热点ntA1762T、ntG1764A、ntG1896A在3组中的发生率差异无统计学意义(P>0.05)。【结论】BCP/PreC/C区基因突变率与HBV DNA定量有关,但未见其与HBV母婴传播免疫失败有明显相关性;也未见tA1762T、ntG1764A、ntG1896A等突变热点与免疫失败的相关性。[Objective] To explore the relationship between gene mutation frequency,mutation hot spots in BCP,PreC/C gene and the immunoprophylaxis failure to prevent hepatitis B virus (HBV) vertical transmission.[Methods] HBsAg-positive and HBV DNA ≥ 1 000 U/mL pregnant women were divided into immune failure group (19 cases) and control group 1 (immune successful and maternal serum HBV DNA ≥ 1.0 ×107 U/mL,48 cases),control group 2(immune successful and maternal serum HBV DNA 〈 1.0 × 107 U/mL,38 cases)according to their neonates whether occur immunoprophylaxis failure and maternal serum HBV DNA level.Mutation frequency and mutational hot spot in BCP/PreC/C gene were detected by PCR amplification technique in three groups.[Results] To compare the quantity of HBV DNA among the three groups,the difference was not statistically significant between immune failure group and control group 1 (P 〉 0.05),but all were higher than the control group 2 (P 〈 0.05).To compare the mutation frequency of BCP/PreS/S gene among the three groups,the difference was not statistically significant between immune failure group and control group 1 (P〉 0.05),but all were lower than the control group 2 (P〉 0.05).Mutation hot spots of ntA1762T,ntG1764A and ntG1896A distribution showed no significant difference among the three groups (P 〉 0.05).[Conclution] Mutation frequency in BCP/PreC/C gene were related to maternal serum HBV DNA level,but not related to immunoprophylaxis failure to prevent HBV vertical transmission,mutation hot spots of ntA1762T,ntG1764A and ntG1896A were not related to immunoprophylaxis failure neither.
