摘 要:目的探讨将管理的动力原则应用于社区护士上门服务绩效考核中的效果。方法 2012年12月—2013年6月东莞市南城区社区卫生服务中心的12个服务站点通过动力原则建立社区护理岗位上门服务绩效考核体系,比较其改革前后各上门服务项目次数、考核得分及家访工作内容的情况。结果改革后的联合义诊或健康教育总次数是改革前的1.4倍,站内义诊或健康讲座总次数是改革前的6.2倍,家访次数是改革前的19.7倍;家访工作内容次数累计是改革前的21.5倍。改革前的家访工作内容以刚性护理为主;改革后的家访工作内容以身体检查为主。各服务站考核总得分的相对比不同,其中水濂站是白马站的4.0倍。各站总家访次数是总联合义诊或健康教育次数的11.6倍,是总站内义诊或健康讲座次数的7.7倍。结论改革后不仅提高了社区护士各上门服务项目的次数和家访工作内容的次数累计,而且丰富了家访工作的内容;不仅能够衡量站与站之间的总体绩效差异,还可以量化个人的绩效差异,使服务内容得到增加,提高了护士工作的积极性。Objective To explore the effect of the application of the dynamic principle of management in performance evaluation systmn of the community nurses' door - to - door services. Methods From December 2012 to June 2013, the 12 health service stations of South City Community Health Service Center in Dongguan established the new performance evaluation system of nurses' door- to -door services based on the dynamic principle of management. The number of service types, the fre- quency of home visits, the score of performance, and the work contents of home vists were collected before and after the estalish- ment of the new system. Results The total number of free clinic consultations and health education held by serveral health serv- ice stations jointly after the retbrm were 1.4 times as many as that before the reform, the total number of free clinic consnhalions and health education held by single community health service station after the reform was 6. 2 times as many as that before the re- form, the numher of home visits increased by 19.7 times after the reform, and the cumulative number of work in home visits in- creased by 21.5 times after the reform. Before the reform, the work contents of home visits were mainly foundmental care, its work contents after the reform were mainly physical examinations. The total performance score of each health service station was different, in which the highest scoree as Shuilian Commnity Health Service Station got was 4. 0 times higher than the lowest scores as the Baima Station got. The total nuraber of home visits of all stations was 11.6 times higher than the number of joint free clinic eonsuhations and health education, and 7. 7 times higher than the number of free clinic consultations and health lectures held by a single station. Conclusion The establishment of the new perfromanee evaluation system could not only increase the number of door- to- door service and home visits done by the nurses in eommnunity health service stations, but also enrich the work con- tents of home
