表浅膀胱癌Pgp与突变p53蛋白表达的相互关系及其共表达的意义

摘　　要：目的　探讨表浅膀胱移行细胞癌中 Pgp与突变 p5 3蛋白表达的相互关系及其共表达情况在预测复发方面的价值。方法　应用免疫组织化学 SP法 ,检测 46例随访 3年以上、术前未经化疗的原发性表浅膀胱移行细胞癌中 Pgp、突变 p5 3蛋白表达情况。结果　 Pgp、突变 p5 3蛋白的阳性表达率分别为 45 .7%、5 8.7% ,Pgp与突变 p5 3蛋白的表达密切相关 (P<0 .0 1) ;其中二者均阳性、其中之一阳性或均阴性表达率分别为 34 .8%、34 .8%、30 .4% ,单因素 Kaplan- Meier生存分析示 Pgp与突变 p5 3蛋白共表达情况与术后复发密切相关 (P<0 .0 5 )。结论　 Pgp与突变 p5 3蛋白共表达情况是判定原发性表浅膀胱移行细胞癌复发的重要指标。Objective To study the relationship between expression of multidrug resistance gene (mdr1) product (P glycoprotein Pgp) and mutant p53 protein and the prognostic value of their co expression in human primary superfical transitional cell carcinomas (TCCs) of bladder.Methods Specimen from 46 untreated patients with superficial bladder TCCs were analysed for expression of Pgp and mutant p53 protein by means of SP immunohischemistry (IHC).Results Pgp and mutant p53 protein were present in 45.7% and 58.7% of TCCs,respectively.The expressions of Pgp were strongly related to the expression of mutant p53 protein ( P <0.01).Co expression of Pgp and mutant p53 protein were found in 34.8% of TCCs,one or no protein positive were found in 34.8% or 30.4%,respectively.A strongly inverse relationship between the co expression of Pgp and mutant p53 protein and recurrence free survival was demonstrated by Kaplan Meier survival analysis ( P <0.05).Conclusion Co expression of Pgp and mutant p53 protein can be used as the important prognostic factor of recurrence of superficial TCCs.
