不同药物胸腔灌注治疗恶性胸腔积液的临床体会

摘　　要：目的比较单独应用顺铂或香菇多糖以及两者联合进行胸腔灌注治疗恶性胸腔积液的疗效及不良反应。方法采用中心静脉导管留置引流术,对120例恶性胸腔积液患者随机分别采用单纯顺铂、单纯香菇多糖或香菇多糖联合顺铂胸腔灌注治疗,治疗4周后按WHO标准评价近期疗效和毒副反应。结果联合组有效率优于单纯顺铂或单纯香菇多糖组;联合组患者的不良反应低于单纯顺铂组,但高于单纯香菇多糖组。结论香菇多糖联合顺铂胸腔灌注是治疗恶性胸腔积液的有效手段。Objective To compare the curative effect and adverse reactions between cisplatin alone and len tinan and combined with intrapleural injection in the treatment of malignant pleural effusion. Methods Using the central venous catheter for drainage of pleural cavity, 120 cases of malignant pleura effusion were randomly intra pleurally injected with cisplatin alone, or lentinan alone or lentinan combined with cisplatin, and the efficacy and tox icity were evaluated 4 weeks after the treatment according to WHO criteria. Results The efficacy was better in the combination group than the cisplatin or lentinan group. The incidence of adverse reactions was lower in the combina tion group than in the cisplatin group, but higher than that in the lentinan group. Conclusion Intrapleural infusion of lentinan combined with cisplatin is an effective method in the treatment of malignant pleural effusion.
