摘 要:目的 研究固醇调节元件结合蛋白-1c(SREBP-1c)基因多态性与非酒精性脂肪性肝病(NAFLD)的关系.方法 选取152例NAFLD患者和146例未患脂肪肝人员,收集血液样本检测血脂水平,采用聚合酶链反应-限制线片段长度多态性(PCR-RFLP)技术分析SREBP-1c基因18号外显子54G/C多态性.结果 NAFLD病例组TC、TG和LDL-C水平均明显高于对照组(P<0.05).两组的SREBP-1c18号外显子54G/C单核苷酸多态性均以GG型的比例最高,两组基因型频率分布有统计学差异(χ2=6.1096,P<0.05),NAFLD病例组中C等位基因频率明显高于对照组(χ2=6.2520,P<0.05).C等位基因携带者患NAFLD的风险是G等位基因的1.6484倍(OR=1.6484,95%CI:1.3233~10.0984).结论 研究对象的血脂水平异常与NAFLD的患病有显著相关性,参与糖代谢、脂代谢的SREBP-1c基因18号外显子54G/C呈现多态性,其C等位基因可能会使个体的NAFLD发病风险增加.Objective To study the correlation between gene polymorphism of sterol regulatory element binding protein lc (SREBP-lc) and non-alcoholic fatty liver disease(NAFLD). Methods 152 patients with NAFLD and 146 persons without fatty liver were selected as the subjects. Their blood samples were collected, and the blood fat levels were detected. Polymerase chain reaction- restriction fragment length polymorphism (PCR-RFLP)was used to analyze the polymorphism of NO. 18 exon 54G/C of SREBP-lc gene. Results The levels of TC,TG, and LDL-C in the NAFLD group were significantly higher than those in the control group ( P 〈 0.05). The proportion of GG type was the highest in the mononucleotide polymorphism of SREBP-lc NO. 18 exon 54G/C in both groups. There were significant differences in the genotypie frequency between the two groups (χ2 = 6. 1096,P 〈 0.05 ). The frequency of C allele genotype in NAFLD group was significantly higher than that in the control group (χ2 = 6. 2520, P 〈 0.05 ). The onset risk degree of NAFLD in the carriers of C allele was 1. 6484 times of that in the carriers of G allele( OR = 1. 6484,95% CI: 1. 3233-10.0984). Conclusion There is a significant correlation between the abnormal blood fat level and the NAFLD morbidity in the subjects. The No. 18 exon 54G/C of SREBP-lc which takes part in the glycometabolism and lipid metabolism presents polymorphism. The C allele may increase the onset risk of NAFLD.
