摘  要:目的探讨全厚皮瓣术后放疗所致放创复合伤早期皮瓣组织内leptin的表达影响。方法选取雄性昆明小鼠,随机分成4组:对照组,创伤组,放射组,放创组,每组24只。所有小鼠背部去毛,创伤组和放创组的小鼠在背部制备出2.5cm×2.0cm大小的矩形随意皮瓣,皮瓣游离端位于髂棘之上1cm,蒂设计在尾端。放创组于术后3天采用5Gy的电子线对小鼠进行局部皮瓣照射。放射后对实验小鼠每日进行大体观察,记录皮瓣愈合时间及被毛再生时间,分别于放射后1d,2d,3d处死各组小鼠8只,取皮瓣游离端进行HE染色及免疫组化研究。结果研究发现leptin及其受体OB-Rb在放射后的浆液腺泡细胞及毛囊根鞘中呈强阳性表达,而在放射后的粘液腺细胞中呈弱阳性表达;在低剂量电子线照射下,放创复合伤局部leptin及其受体OB-Rb在1d-3d的表达量逐渐增加,放创组分别高于单创组,高于放射组和对照组,放射组高于对照组(P<0.05)。结论电离辐射后早期皮瓣组织浆液性腺泡细胞及毛囊根鞘内leptin及其受体OB-Rb反应性增高,提示浆液性腺泡细胞及毛囊根鞘是电离辐射的靶细胞,参与了放创复合伤的早期修复过程,通过释放分泌leptin参与放创复合伤的早期愈合。Objective Our aim is to discuss the expression of Leptin in the full-thickness flap tissue,which was damaged by postoperative radiotherapy and surgical trauma.Methods Male Kunming mice were selected and randomly divided into four groups:Control Group,Trauma Group,Rodiation Group and Radiation Trauma Group,each group 24 mice.All the mice were hair-cut,we derived a type of 2.5 cm× 2.0 cm rectangle flap on the back of the mice in both trauma group and radiation trauma group.Free end of the flap is 1.0 cm above the iliac spine,and the pedicle was designed at the end of the flap.Mice belonged to the radiation trauma group were given a 5Gy irradiation of electrical beam 3 days after the surgery.Then we daily valued the epithelium healing condition of mice in each group and record the flap healing and clothing hair regenerating condition,8 mice of the each group were killed respectively ld,2d,3d after radiation,and the free side of flap was obtained for HE staining and immunohistochemical staining.Results The Leptin and its antibody-OB-Rb expressed in serous acinar cells of the flap tissue after electrical radiation was strongly promoted,which in mucus gland cells was weakly positive expression; Ob-Rb in serous acinar cells after radiation was strongly positive expression,and in mucous gland cells after radiation was weakly positive; under low doses of electron irradiation,the leptin expression of radiation trauma group in 1d-3d increased gradually,and 1 d,2 d,3 d leptin at the same time point in radiation trauma group was respectively higher than the trauma group,the radiation group and the control group,the radiation group was higher than the control group.Conclusion The reactivity of leptin and its antibody ObRb in early flap serous acinar cells after ionizing radiation increased,the result suggested that the serous acinar cells are the target cells of ionizing radiation,whose function of releasing Leptin can promote the early repairing process of the surgical-radiotherapy trauma.
