多烯不饱和脂肪酸在结直肠癌患者和健康人群状况分析

摘　　要：目的通过检测结直肠癌患者和健康人群血红细胞膜多烯不饱和脂肪酸(PUFA)的水平,分析比较PUFA在2组人群的代谢差异及对结直肠癌的影响。方法随机选择解放军总医院健康体检156例为健康人群组,结直肠癌患者63例为结直肠癌组。检测和分析2组人群血红细胞膜PUFA的水平,并按结直肠癌的不同分期、分化、大小、有无淋巴结转移等因素分类,比较PUFA的代谢状况。结果总ω-6 PUFA在结直肠癌组低于健康人群组[(25.77±3.21)%vs(26.86±1.65)%,P<0.05)],其主要组分花生四烯酸(AA)也低于健康人群组[(13.77±2.49)%vs(14.77±1.52)%,P<0.05]。总ω-3 PUFA在结直肠癌组低于健康人群组[(5.02±1.32)%vs(6.08±1.26)%,P<0.05],其主要组分二十碳五烯酸、二十二碳六烯酸也低于健康人群组[(0.31±0.27)%vs(0.39±0.24)%,P<0.01;(3.53±1.22)%vs(3.85±1.00)%,P<0.05]。肿瘤大小(<5 cm和≥5 cm)相比,AA和总ω-6 PUFA差异有统计学意义[(13.11±2.91)%vs(14.44±1.81)%,P<0.05;(27.02±4.54)%vs(28.76±3.08)%,P<0.05]。结论健康人群和结直肠癌患者主要的PUFA水平存在着差异,结直肠癌患者大部分ω-3 PUFA和ω-6 PUFA低于健康人群。结直肠癌的肿瘤大小可能对PUFA代谢有一定的影响。Objective To analyze the metabolic difference of PUFA in colon cancer patients and in health controls by testing levels of polyunsaturated fatty acids（PUFA） on membrane of red blood cells. Methods One hundred and fifty-six healthy persons and 63 colon cancer patients in hospital were enrolled. PUFA level of blood in the two groups was detected,and the PUFA level was tested in relation to tumor stages,differentiation,size,status of lymph node metastasis. Results Total level of ω- 6 PUFA in colon cancer patients was lower than that in health persons [（25.77 ± 3.21）% vs（26.86 ± 1.65）%,P〈0.05]. Main components（arachidonic acid,AA） contents in colon cancer patients was lower than that in health person [（13.77 ± 2.49）% vs（14.77 ± 1.52）%,P〈0.05]. Total level of ω3- PUFA in colon cancer patients was lower than that in health person [（5.02 ± 1.32）% vs（6.08 ± 1.26）%,P〈0.05]. Main components（Eicosapentaenoic acid,EPA and Docosahexaenoic acid,DHA） contents in colon cancer patients was lower than that in health person（0.31 ± 0.27）% vs（0.39 ± 0.24）%,P〈0.01;（3.53 ± 1.22）% vs（3.85 ± 1.00）%,P〈0.05]. There was a significant difference in size of tumors（ 5 cm versus ≥ 5 cm）,contents of AA and ω- 6 PUFA [（13.11 ± 2.91）% vs（14.44 ± 1.81）%,P〈0.05;（27.02 ± 4.54）% vs（28.76 ± 3.08）%,P〈0.05]. Conclusion There is a significant difference in main ω- 3 PUFA and ω- 6 PUFA between the healthy population and colon cancer patients. The size of colon cancer may influence the metabolic level of PUFA.
