健脾益气法对胃癌术后患者营养状况和免疫功能的影响

摘　　要：目的观察鼻饲健脾益气中药对胃癌患者术后营养状况和免疫功能的影响。方法胃癌患者术后第3天起每日予常规肠内营养,随机分为两组,对照组(29例)维持肠内营养,观察组(31例)加用健脾益气中药煎剂100ml鼻饲,至术后第7天。两组均检测术前1天及术后第1、5、8天血浆总蛋白(TP)、清蛋白(Alb)、前清蛋白(PA)、T淋巴细胞亚群(CD3+、CD4+、CD8+、CD4+/CD8+)、免疫球蛋白(IgG、IgA、IgM)水平。结果与对照组比较,观察组术后第8天血浆PA、CD4+、CD4+/CD8+、IgG、IgA均显著增高(P<0.05,P<0.01)。结论术后予以健脾益气方能更好地改善胃癌患者营养状态和免疫功能。Objective To evaluate the effect of nasal feeding spleen-strengthening and Qi-invigorating formula on nutritional and immune status in postoperative patients with gastric carcinoma. Methods Patients who received surgeries for their gastric cancers were enrolled and randomly divided into two groups which were observation group and control group. Patients in two groups were provided postoperative enteral nutrition support from the third day to the seventh day after operation. Additionally, patients in observation group were administered with 100 ml spleen-strengthening and Qi-invigorating formula from enteral feeding tube. Plasma levels of protein（TP, Alb, PA ）, T-lymphocyte subsets （CD3 ＋ , CD4 ＋ , CD8 ＋ , CD4＋ /CD8＋ ） and immune globulin（IgG, IgA, IgM） were examined on the first day before the operation and the first, the fifth and the eighth day after the operation. Results Plasma level of albumin, CD4＋ ,CD4＋/CD8＋ ,IgG and IgA in the patients of observation group were significantly reduced compared with those in the control group（P〈0. 05,P〈0. 01）. Conclusion Nasal feeding spleen-strengthening and Qi-invigorating formula can effectively improve nutritional and immune status in postoperative patients with gastric carcinoma.
