鱼油脂肪乳剂对胃肠肿瘤患者术后单核细胞功能的影响

摘　　要：目的探讨鱼油脂肪乳对胃肠肿瘤患者术后单核细胞功能的影响。方法64例胃肠道肿瘤患者采用信封法随机分为对照组（32例）和研究组（32例），两组患者均给予含中长链脂肪乳剂肠外营养支持，研究组加用鱼油脂肪乳。于患者术后第1、3、6天分别检测人类主要组织相容性抗原-DR、单核细胞培养液白细胞介素（IL）-1β、IL-6、IL-10、肿瘤坏死因子（TNF）-α的变化。结果术后连续5d肠外营养支持后，研究组IL-1β[（1818±213）pg／L]、IL-6[（415±71）pg/L]、TNF-α[（2194±350）pg／L]含量较对照组[IL-1β：（2292±289）pg／L；IL-6：（791±163）pg／L；TNF—α：（2509±507）pg/L]显著降低（P=0．027，P=0．001，P=0．036），但两组IL-10的差异无统计学意义。研究组外周血单核细胞人类主要组织相容性抗原-DR表达水平（76．6±15．2）％较对照组（62．3±16．8）％显著升高（P=0．029）。结论使用添加鱼油脂肪乳的肠外营养有利于改善胃肠肿瘤患者术后的单核细胞功能。Objective To investigate the effect of fish oil parenteral supplementation on monocyte function in gastrointestinal cancer patients. Methods Sixty-four gastrointestinal cancer volunteers were equally randomized into study group and control group. Both groups received conventional lipid emulsion, and the study group was added with fish oil parenteral supplementation for 5 days. Blood samples were collected on the 1 st, 3rd, and 6th post-operative days to detect the interleukin （IL） -1β, IL-10, IL-6, tumor necrosis factor-α, and human leukocyte antigen-DR. Results After 5 days of continuous management, the IL-1β, IL-6, and tumor necrosis factor-α levels in the study group became significantly lower than those in the control group [ （ 1 818 ± 213）pg/Lvs （2 292±289）pg/L, P=0.027; （415±71）pg/Lvs （791±163）pg/L, P=0.001; （2 194±350 ） pg/L vs （ 2 509 ±507 ） pg/L, P = 0. 036, respectively ） , whereas the level of IL- 10 by mononuelear leukocytes in two groups showed no significant difference. The expression of human leukocyte antigen -DR was significantly increased in both groups, but was significantly higher in the study group than in the control group [ （76.6±15.2）% vs （62.3±16.8）%, P=0.029]. Conclusion Fish oil helps improve postoperative monocyte function in patients with gastrointestinal tumor.
