晚发性精神分裂症患者双侧额叶指数的对照研究

摘　　要：目的对照观察晚发性精神分裂症(LOS)患者双侧额叶指数(FI)的改变。方法 84例LOS患者进行头颅CT扫描后、测定双侧FI,与84例性别、年龄匹配的健康人进行对照,并分析FI与LOS患者临床特征的关系。结果①患者组左、右侧FI值均显著高于对照组的同侧值(t=2.117,2.054;P<0.05);②单因素分析显示,患者组左侧FI与年龄(r=0.313,P<0.05)、PANSS总分(r=0.270,P<0.05)、阳性因子分(r=0.284,P<0.05)正相关;右侧FI仅与年龄正相关(r=0.298,P<0.05);③线性回归分析显示,患者组左侧FI与年龄、PANSS量表阳性因子分正相关(β=0.073、0.006,P<0.01);右侧FI仅与年龄正相关(β=0.065,P<0.01)。结论晚发性精神分裂症患者可能存在双侧额叶的萎缩,且左额叶改变与临床特征更有内在联系。Objective To explore the morphological changes of frontal lobe in late -onset schizophrenia（LOS）patients.Methods Eighty-four LOS patients and sex ,age-matched healthy controls were scanned with computed tomography to calculate bi -frontal index （ FI） for analysis .Results Either of left or right frontal index in the LOS patients were significantly higher than those in normal controls （t=2.117,2.054;both P〈0.05）.In univariate analysis,there were significantly positive correlations in LOS patients between FI and age（r=0.313,P〈0.05）,score of PANSS（r=0.270,P〈0.05）,score of positive symptom（r=0.284,P〈0.05）in left frontal lobe;and only between FI and age in right frontal lobe （r=0.298,P〈0.05）.Multiple linear regression analysis showed that left FI was posi-tively associated with age（β=0.073,P〈0.01）,score of positive symptom（β=0.006,P〈0.05）,and right FI was only positively relat-ed to age（β=0.065,P〈0.01）.Conclusion The atrophy of the frontal lobe might present in late -onset schizophrenia,which has more relationships with clinical characteristics in left than that in right frontal lobe .
