大肠良、恶性病变的癌胚抗原表达及其价值

摘　　要：应用 CEA 单克隆抗体,借助 BA 及 IGSS 染色技术,对20例大肠正常粘膜、87例息肉及34例癌组织进行免疫组化研究,结果显示:多数大肠正常粘膜 CEA 为阴性,少数为弱阳性,其 CEA的分布呈极向性.多数大肠癌组织 CEA 呈较强阳性,CEA 的分布失去极向性,癌组织 CEA 的分布类型与大肠癌患者的顶后有密切关系。大肠息内尤其是腺瘤的 CEA 表达介于前两者之间,腺瘤组织异型程度越重,其 CEA 表达越近似于大肠癌,表明 CEA 免疫组化染色可辅助病理形态学来判断腺瘤癌变的危险性,对大肠癌二级防治具有一定意义.本文 IGSS 染色的结果与 BA 法相近似,IGSS 法具有较多优点,值得推广应用.The carcinoembryonic antigen(CEA)expressions of normal mucosa(20 cases),polyps(87 cases)and carcinomas(34 cases)of large bowel were detected immunohistochemically using CEA McAb with HRP-Labeled Avidin Biotin Technique(BA)as compared with Immunogold Silver Staining(IGSS)method.In most normal mucosa,CEA was negative,only in small numbers of them weak positive was found;CEA was expressed in a polar manner.Stronger positive staining was ex- pressed in most carcinomas,the polarity of CEA was lost.The distribution patten of CEA was associated with prognosis of carcinomas.CEA expressions in polyps were in the middle of the two above.As the degree of dysplasia of adenomas increased,the CEA expressions were more closely similar to those in the carcinomas.CEA immunohistochemical staining could be used for assessing the risk of malignant change of adenomas.The results of BA were similar to IGSS,but IGSS may be a new method for be- ing used widely.
