摘  要:菲律宾某市贫困人口中孕妇和乳母的营养不良 (BMI<18.5 )及缺铁性贫血 (Hb <11g/dL)患病率很高 ,通过分析营养调查资料 ,建立了其原因模型 (causalmodel) :基本原因 (basiccauses)→潜在原因 (underlyingcauses)→直接原因 (immediatecauses) ,依据这个原因层次制定了包括“营养食品补充”、“微营养素补充”、“营养教育”和“改善就业”的一个综合干预计划 (HealthyMommy ,HealthyBaby) ,旨在改善贫困妇女和乳母的营养状况 ,以减少低体重婴儿出生率和促进婴幼儿生长发育。干预后 ,孕妇平均增加体重 3.72± 1.45kg(P <0 .0 5 ) ,乳母平均增加体重 2 .0 2± 1.2 8kg(P >0 .0 5 ) ,孕妇组有 17人 (17/ 6 9)BMI≥ 18.5 ,乳母组有 8人 (8/ 45 )BMI≥ 18.5 ;孕妇组有 38人 (38/ 6 9)Hb≥ 11g/dL ,乳母组有 2 8人 (2 8/ 45 )Hb≥ 11g/dL ;受试者基本营养知识水平和食物消费行为改善明显 ;但“改善就业”措施未取得成效。The 1998 National Nutrition Survey conducted by the Food and Nutrition Research Institute showed that the prevalence rates of undernutrition and anemia were higher among pregnant and lactating women of poor population in a city of Philippines. Therefore we analyzed nutrition situation of the city, identified nutritional problems and established a causal model, whereby a integrative intervention project (Healthy Mommy, Healthy Baby), which consisted of supplementary feeding, micronutrients supplementation, nutrition education and employment supply aimed to ensure the birth of healthy babies by reducing the prevalence of undernutrition and anemia among pregnant and lactating women, was formulated. 69 pregnant women and 45 lactating women with undernutrition and anemia were selected to participate in the project. After completing of the project, average weight gain of pregnant group was 3.72 ±1.45kg (P<0.05), of which 17 women (17/69) reached the level of BMI ≥18.5; and average gain of lactating group was 2.02± 1.28 (P>0.05), of which 8 ones (8/45) reached the level of BMI≥18.5. As to Hb concentration, 38 pregnant women (38/69) and 28 lactating ones (28/45) reached the level of Hb≥g/dl. Nutrition knowledge and dietary behavior were significantly improved, but supplying of employment did not reach the expected effects among all subjects.
