免疫球蛋白超家族分子CD54在各种恶性淋巴增殖性疾病的表达

摘　　要：目的　为了探讨免疫球蛋白超家族分子CD5 4(细胞间粘附分子 1)在恶性淋巴增殖性疾病的表达及其意义。方法　用免疫荧光方法检测 2 4例急性淋巴细胞白血病 (acutelymphoblasticleukemia ,ALL) ,15例多发性骨髓瘤 (multi plemyeloma ,MM) ,2 6例非霍杰金氏淋巴瘤 (non Hodgkinslymphoma ,NHL) ,4例慢性淋巴细胞白血病 (chroniclymphocyticleukemia ,CLL) ,4例霍杰金氏淋巴瘤 (Hodgkinslymphoma)及 16例正常人造血干细胞CD+ 3 4细胞表面CD5 4的表达。结果　( 1)除多发性骨髓瘤、霍杰金氏淋巴瘤外 ,非霍杰金氏淋巴瘤、慢性淋巴细胞白血病、急性淋巴细胞白血病细胞膜表面CD5 4表达低于正常造血细胞CD+ 3 4细胞 (分别为 12 .9± 5 .7%、10 .2± 2 .8%、11.2± 5 .8%VS47.7± 12 .1%)。 ( 2 )细胞间粘附分子 1与细胞类型有关 ,在T ALL表达明显低于B ALL表达。 ( 3) )细胞间粘附分子 1与非霍杰金氏淋巴瘤 (NHL)部分临床特征有相关性 ,有结外病变、骨髓浸润、Ⅳ期患者表达水平下降 (P <0 .0 5 )。结论　不同的恶性淋巴增殖性疾病粘附分子CD5 4的表达存在不同的改变并与NHL临床特征有关。Objective To investigate the expression of immunoglobulin supergene family adhesion molecule (CD54) in malignant lymphoproliferative diseases and its correlation to certain clinical features.Methods Monocytic cells of bone marrow samples from 24 cases with acute lymphoblastic leukemia(ALL),15 cases with multiple myeloma(MM),26 cases with non-Hodgkins lymphoma(NHL),4 cases with chronic lymphocytic leukemia(CLL),4 cases with Hodgkins lymphoma(HD) and 16 healthy persons were measured by immunological fluorescence analysis for CD45 expression.Results (1)Except MM?HD,NHL?CLL?ALL blast cells expressed relatively lower levels of CD54 than normal hematopoietic CD + 34 cells(12.9±5.7%?10.2±2.8%?11.2±5.8%,respectively VS 47.78±12.1%).(2) CD54 was lower expressed on T-ALL than on B-ALL(3)A significant positive correlation could be demonstrated between low CD54 expression and extranodular involvement?bone marrow infiltration?Ⅳ stage at the time of diagnosis for NHL(P<0.01).Conclusion Our observations suggest that there are abnormalities in the expression of cell adhesion molecules (CD54) in malignant lymphoproliferative diseases which has an impact on the clinical characteristics of non-Hodgkins lymphoma.
