牟重临《诊余思悟一得集》临诊学术思想探析

摘　　要：[目的]继承与发扬牟重临老师的学术思想及其临床经验。[方法]通过研究牟师著作《诊余思悟一得集》,分析书中所论述牟师的学术思想特色以及临床独特的诊疗经验,并阐发其中具有突出的创新性内容。[结果]牟师认为发展中医临床既要遵循传统辨证,又主张知常达变,遵古而不泥古,临床上勤用反向思维。创造性地将现代科学概念引入中医理论认识,如采用几何图形解读《伤寒论》六经方证;将热力学"熵"定律精神与《内经》学术思想和东垣脾胃学说对照,提出两者内涵的相似点;深入论述东垣脾胃学说的临床贡献;对方剂的触类旁通使用,对古代"痰病"治法的推广运用,均在临床上收到良效。[结论]从《诊余思悟一得集》中所展现的牟重临老师知常达变、中西合参、师宗东垣的学术思想及独到的临床见解,体现了中医传承中的创新,具有重要的学术价值和临床意义。[Objective]Inherit and carry forward the academic thinking and clinical experience of MOU Chonglin.[Method] Through the study of the works of MOU Chonglin, we analyze the academic ideological features of MOU teacher and the unique clinical experience of diagnosis and treatment in the book,and elucidate the outstanding innovative content in this book.[Results] MOU believes that the development of TCM clinical practice not only follows the traditional syndrome differentiation, but also emphasizes the change of knowledge. The concept of modern scientific concept is creatively introduced into the theory of traditional Chinese medicine, such as the use of geometric figure to interpret the six meridians of typhoid fever, and the similarities between the spirit of entropy law of thermodynamics and the theory of the Nei Jing and the theory of the spleen and stomach of LI Dongyuan, and the clinical contribution of the theory of the spleen and stomach of LI Dongyuan; The use of each other’s analogy has received good results in prescriptions and ancient "phlegm" treatment.[Conclusion] MOU Chonglin’s academic thoughts and clinical experience have many unique insights, and have deep research significance and clinical practical value.
