六君子汤合半夏白术天麻汤化裁对急性缺血性脑卒中患者BDNF、VEGF、MMP-9水平的影响

摘　　要：目的:观察六君子汤合半夏白术天麻汤化裁治疗急性缺血性脑卒中患者的临床疗效,并探讨其对患者血清BDNF、VEGF、MMP-9水平的影响。方法:将沧州市中心医院就诊的172例缺血性脑卒中患者随机分为对照组和观察组各86例。对照组患者给予常规西药治疗;观察组患者则在对照组基础上加用六君子汤合半夏白术天麻汤化裁而成的汤剂内服,两组均连续用药4周。通过中医证候积分变化对比两组治疗的临床效果,评定治疗前后神经功能缺损程度,监测治疗前后患者血液流变及血清脑源性神经营养因子(BDNF)、血管内皮生长因子(VEGF)、基质金属蛋白酶-9(MMP-9)水平的变化。结果:观察组有效率高达93.0%,显著高于对照组72.1%(P<0.05);观察组患者经治疗后中医证候积分显著低于对照组(P<0.05);神经功能NIHSS评分显著低于对照组(P<0.05);血液流变学指标较对照组明显降低(P<0.05);血清中BDNF、VEGF水平对照组明显升高,而MMP-9水平则显著降低(P<0.05)。结论:六君子汤合半夏白术天麻汤化裁利于改善急性缺血性脑卒中患者的临床症状,减轻神经功能损伤,减轻脑血液灌注损伤,值得临床深入研究。Objective:To investigate the clinical efficacy of treating ischemic stroke patients with Liujunzi Decoction and Banxia Baizhu Tianma Decoction.Methods:A total of 172 patients were randomly divided into a control group(86 cases) and an observation group(86 cases).Patients in the control group were given conventional western medicine while patients in the observation group were treated with Liujunzi Decoction and Banxia Baizhu Tianma Decoction,4 weeks for a treatment course.The paper was aimed at comparing the effectiveness,evaluating the syndrome integral,observing the improvement of degree of nerve function defect,and monitoring the changes of hemorheology,BDNF,VEGF and MMP-9 levels.Results:The efficiency of patients in the observation group was 93.0%,which was higher than the control group′s 72.1%(P<0.05).The symptoms scores for the patients in the observation group were lower than that of the control group(P<0.05).The NIHSS scores were lower than that of the control group(P<0.05).The blood rheology indicators for patients in the observation were lower than that of the control group(P<0.05).The BDNF and VEGF levels for patients in the observation were higher than that of the control group,while the MMP-9 level of the patients in the observation group was lower than that of the control group after treatment(P<0.05).Conclusion:The therapy is helpful to improve the clinical symptoms of acute ischemic stroke patients,reduce the nerve function injury and the cerebral blood perfusion injury,worthy of further research.
