摘 要:目的探索胜任力培养模式在血液科住院医师规范化培训中的作用。方法以2015年6月-2017年12月在某院血液科轮转的规范化培训的住院医师共50人为研究对象,按随机分组的方法分为两组:实验组25人应用胜任力教学培养模式、对照组25人应用传统三级医师教学模式。培训结束后,两组规培医师采用360度评价法进行全方位评估。结果实验组和对照组在医学知识、临床思维、临床技能、职业道德、团队协作和医患沟通方面比较,差异均具有统计学意义,P值分别为0.02、0.01、0.03、0.03、0.02、0.04;考试成绩级别结果显示在医学知识、临床思维、临床技能、医患沟通和等级为5级的规培医师比例,实验组和对照组差异均具有统计学意义,P值分别为0.00、0.01、0.00、0.04;护士对实验组和对照组在团队协作、职业道德方面评价结果差异具有统计学意义,P值分别为0.01、0.01、0.04;患者对实验组和对照组在团队协作、职业道德方面评价结果差异具有统计学意义,P值分别为0.04、0.03、0.02。结论胜任力培养模式可明显提高规范化培训住院医师的职业素养。Objective To explore the role of competency training in the standardized training of residents in Department of Hematology.Methods A total of 50 standardized training of the resident doctors(Trainer) from June 2015 to December 2017 were randomly divided into two groups:experiment group(competency teaching training mode) and control group(traditional teaching mode).They were evaluated by the 360-degree evaluation method after the training.Results There were significant differences between experimental group and control group in medical knowledge,clinical thinking,clinical skills,professional ethics,teamwork and doctor-patient communication.The difference was statistically significant(P=0.02,0.01,0.03,0.03,0.02,0.04,respectively).The results of test scores in experimental group were significantly higher in medical knowledge,clinical thinking,clinical skills,doctor-patient communication and the proportion of physicians in grade 5 than those in control group(P= 0.00,0.01,0.00,0.04,respectively).Nurses(P=0.01,0.01,0.04,respectively)and patients(P=0.04,0.03,0.02,respectively) had significant difference in team cooperation and professional ethics between experimental group and control group.Conclusion Competency training mode can significantly improve the professionalism of standardized training residents.
