摘 要:目的探讨维生素E(Vitamin E,VE)水平与宫颈癌发病风险之间的关系。方法检索1980年至2017年5月期间关于VE与宫颈癌发病关系研究的文献,使用Stata12.0对符合纳入标准的文献按照不同指标进行定量合成分析。结果最终纳入17篇文献,包括了2631例宫颈癌病例; meta分析结果显示:(1)膳食VE摄入水平的最高组比最低组与宫颈癌发病风险的关联有统计学意义(OR,0.64;95%CI,0.46-0.90),且存在一种线性剂量反应关系(Pfornon-linearity:0.45)。(2)血清VE水平的最高组比最低组与宫颈癌发病风险的关联有统计学意义(OR, 0.69; 95%CI, 0.57-0.85);亚组分析结果显示,在病例对照研究中高水平的血清VE是保护因素,高水平的血清VE在北美洲组和亚洲组是保护因素。敏感性分析和发表偏倚检验结果显示,本研究Meta分析结果稳定,不存在发表偏倚。结论增加膳食VE摄入量可以降低宫颈癌的发病风险。Objectives We conducted a meta-analysis to evaluate the relationship between vitamin E and risk of cervical neoplasia(CN). Methods The fixed or random effect model was selected based on heterogeneity. Meta-regression was used to explore potential sources of between-study heterogeneity. Publication bias was evaluated using the Begg’s test. Dose-response relationship was assessed by restricted cubic spline model. Results Dietary vitamin E intake and high serum tocopherol levels were significantly associated with decreased risk of CN. The pooled ORs(95%CI) were 0.64(95%CI, 0.46-0.90) and 0.69(95%CI, 0.57-0.85), respectively. The findings from dose-response analysis showed a linear association between dietary vitamin E intake and risk of CN. The risk of CN decreased by 3%(OR=0.97, 95% CI, 0.95-0.99) for every 1 mg/day increment of dietary vitamin E intake. Conclusion The findings of this meta-analysis indicate that dietary vitamin E intake, and high level of serum tocopherol are significantly associated with reduced CN risk.
