福建地区人乳头瘤病毒感染与口腔癌关系的病例对照研究

摘　　要：目的 研究人乳头瘤病毒（HPV）感染与口腔癌的关联. 方法 采用病例对照研究,病例为福建医科大学第一附属医院口腔颌面外科经病理确诊的新发病例75例,对照为按性别年龄频数匹配的社区人群75例.病例组采集新鲜癌组织,对照组采集口腔黏膜细胞,提取组织DNA,运用HPV基因微阵列分型方法检测21型HPV.采用非条件Logistic回归分析HPV感染与口腔癌发病风险的调整比值比（OR）及95％可信区间（95 ％CI）.结果 HPV16/18感染增加了口腔癌的发病风险,其调整的OR为12.457 （95％CI：1.325～117.117）.HPV感染与年龄、性别、文化程度、吸烟、饮酒有关（P＜0.05）. 结论 HPV16/18感染可能是福建地区口腔癌的危险因素,HPV16/18感染与年龄、性别、文化程度、吸烟、饮酒有关.Objective To study the association between HPV infection and oral cancer.Methods A case control study was designed with 75 cases and 75 controls.Patients with pathologic confirmation were recruited from the First Affiliated Hospital of Fujian Medical University.Control subjects from specific communities were enrolled,matched to cases on age and gender.Fresh specimens of oral cancer and mucosal tissues of control group were collected,separately.The genomic DNA was extracted from the two groups of samples,and HPV gene microarray method was employed to detect 21 HPVs.Unconditional logistic regression model was performed to calculate odds ratios （ORs） and 95% confidence intervals （CIs） of HPV16/18 infection in oral cancer.Results HPV16/18 infection significantly increased the riskof oral cancer with OR 12.457（95%CI：1.325～117.117）; HPV infection was correlated with age,gender,education level,smoking and drinking（P〈0.05）.Conclusions HPV16/18 infection may be the risk factor of oral cancer and HPV infection is related to age,gender,education level,cigarette smoking and alcohol drinking in Fujian Area.
