摘  要:目的 检测大肠癌组织中KRAS与磷脂酰肌醇-3-激酶催化亚单位oα(PIK3CA)突变,探讨其与大肠癌转移、复发之间的关系.方法 收集大肠癌标本94例,提取DNA后行KRAS和PIK3CA基因测序,分析其临床病理特征,探讨两者之间的关系;并对患者进行为期3年的随访,分析基因突变与转移、复发的关系.结果 KRAS基因突变31例,远处转移率高于野生型(61.3%比34.9%);PIK3CA基因突变15例,远处转移率高于野生型(73.3%比38.0%);两者共同突变7例,均未突变55例,共同突变者转移率高于均未突变者(71.4%比29.1%);术后3年随访KRAS基因突变型转移率和复发率均高于野生型(93.1%比69.5%,65.5%比39.0%),PIK3CA基因突变型转移率和复发率亦均高于野生型(93.3%比74.0%,73.3%比42.5%);上述结果差异均有统计学意义(P<0.05).结论 大肠癌组织中KRAS和PIK3CA基因突变型远处转移率和复发率高,对大肠癌的远处转移和复发可能有一定的预测价值.Objective To detect KRAS and Phosphatidylinositol 3-kinase catalytic alpha (PIK3CA) gene mutations in colorectal cancer,and to assess their relationships with metastasis and relapse.Methods Ninety-four CRC tumor specimens were investigated,and KRAS/PIK3CA mutations were detected by gene sequencing analysis after genomic DNA extracted.The mutation results were analyzed with the corresponding clinical pathological data,especially with metastasis,and 3-year follow up data were also obtained for metastasis and recurrence.Results Thirty-one (33.0%) had KRAS gene mutations,and there was an increased incidence of metastasis with KRAS mutant when compared with wild-type (61.3% vs.34.9%).Fifteen (16.0%) had PIK3CA gene mutations,and there was also an increased incidence of metastasis as compared with wild-type (73.3% vs.38.0%).Seven cases (7.4%) had both KRAS and PIK3CA mutations,and displayed an increased metastasis when compared with neither KRAS nor PIK3CA mutations.Three-year follow-up data demonstrated an increased incidence of metastasis and recurrence with KRAS mutant (93.1% vs.69.5%,and 65.5% vs.39.0%),and PIK3CA showed the same trend (93.3% vs.74.0%,and 73.3% vs.42.5%),P<0.05.Conclusion KRAS and PIK3CA mutants were related with metastasis and recurrence,and may have a predictive value for metastasis and recurrence.
