摘 要:目的:探讨鱼油对胃肠肿瘤病人术后炎性反应及临床预后的影响。方法:64例胃肠道肿瘤病人随机分为对照组(32例)和观察组(32例),两组病人均给予PN治疗104.6kJ·kg-1·d-1,观察组加用鱼油。于病人术后d1、d3、d6分别检测中性粒细胞计数(NGC)、血清C反应蛋白(CRP)、血清白介素-1β(IL-1β)、血清白介素-6(IL-6)、血清白介素-10(IL-10)、肿瘤坏死因子-ɑ(TNF-ɑ)的变化。统计两组病人全身炎症反应综合征(SIRS)、术后感染发生率、其他术后并发症发生率、死亡率的变化。结果:术后两组血清IL-1β、CRP、NGC的含量明显变化,观察组指标明显低于对照组(P<0.05),观察组SIRS明显低于对照组(P<0.05)。结论:鱼油有利于改善胃肠肿瘤病人术后的炎性反应及临床预后。Objective : To investigate the influence of fish oil-based (ω-3) (study group)vs conventional (ω-6)lipid emulsion (control group)on modulating inflammation status and prognosis improve in patients of gastrointestinal tumor. Methods:Sixty-four gastrointestinal cancer volunteers were randomized to receive an ω3 lipid emulsion or a conventional (ω-6)lipid emulsion for 5 days. Blood samples were gained on the morning of day 1, on the morning of day 3 and day 6 after operation respectively to measure IL-1β, IL-10, IL-6, TNF-α and The neutrophilic granulocyte count (NGC), Serum c-reactive protein (CRP). The incidence of of two groups of patients with systemic inflammatory response syndrome (SIRS), postoperative infection, and other postoperative complications and mortality were surveyed. Results :NGC and C RP, IL-1β of both groups were significantly reduced, and the study group was Significantly lower than control group (P〈0.05). SIRS in the study group was lower than control group(P〈0.05 ). Conclusions: Fish oil helps improve postoperative inflammatory response and clinical outcomes for patients with gastrointestinal tumor.
