我国乡镇卫生院卫生人才继续教育现状及需求分析

摘　　要：目的了解我国乡镇卫生院卫生人才继续医学教育现状及需求,并提出对策与建议。方法 2011年4—9月,采用多阶段分层随机抽样法,选取北京、浙江、山西、安徽、贵州和云南6个省市乡镇卫生院的医务人员进行问卷调查。调查内容包括:基本情况、继续教育现状、继续教育存在的问题及继续教育的需求等。结果乡镇卫生院医务人员的继续教育机构仍然以县级医疗机构(42.4%,305/719)和区县行政部门及其委托单位(29.6%,213/719)为主;继续教育形式以短期多次培训(56.0%,380/678)为主。乡镇卫生院医务人员认为继续教育中存在的主要问题有:教学缺乏实际操作技能的训练(66.1%,475/719)、培训或学习时间短(54.2%,390/719)、教学内容针对性不强(44.1%,317/719)等。乡镇卫生院医务人员认为最需要补充的专业知识有:全科医学知识(82.5%,593/719)、西医专业知识(44.1%,317/719)、疾病预防控制(38.7%,278/719);而对继续教育形式的需求:42.9%(255/595)的医务人员希望能组织专家教授到农村实地培训,33.4%(199/595)希望能参加定期集中专题学习,而仅8.4%(50/595)希望能短期多次培训。结论目前乡镇卫生院对其卫生人才的继续教育尚没有形成完善的体系,且存在着继续教育形式单一、培训与需求脱节、缺乏针对性等问题。对于乡镇卫生院卫生人才的培养,要从学历教育、毕业后教育和继续医学教育三个方面三轨并进。Objective To understand the current situation and demand of health professionals’ continuing medical education in township health centers and put forward countermeasures and suggestions. Methods Multistage and stratified random sampling method was used to select some medical staff in township health centers in Beijing,Zhejiang,Shanxi,Anhui,Guizhou and Yunnan from 4—9,2011. They were surveyed by questionnaires on general information,continuing education situation,the existing problems and demands. Results The continuing education institutions were mainly county level ones（ 42. 4%,305 /719） and district and county administrative departments as well as their commission institutions（ 29. 6%,213 /719）. The training was mainly short term ones repeated for many times（ 56. 0%,380 /678）. The existing problems： lack of practical operative skill training（ 66. 1%,475 /719）,short training or study periods（ 54. 2%,390 /719） and weak pertinence of content（ 44. 1%,317/719） and so on. Professional knowledge they mostly needed to equip： general medicine knowledge（ 82. 5%,593 /719）,professional knowledge of western medicine（ 44. 1%,317 /719） and disease prevention and control（ 38. 7%,278 /719）,etc. Their demand of training way： 42. 9%（ 255 /595） of the medical staff hoped that experts and professors could give field training in villages and counties; 33. 4%（ 199 /595） hoped to take part in regular and centralized specific topic learning; only 8. 4%（ 50 /595） wanted short- term training repeated for several times. Conclusion The system of continuing medical education training of health professionals in township health center was still not complete. There still have such problems as lack of targeted training,disjunction of training and demand,lack of diversity of training forms,etc. We propose to cultivate health professionals in townshop hospitals from academic education,postgraduate education and continuing medical education all together.
