绿茶与胃癌风险队列研究的Meta分析

摘　　要：目的纳入前瞻性的队列研究,系统评价绿茶与胃癌发生风险相关性。方法计算机检索Cochrane Library、PubMed、Embase、中国生物医学文献数据库（CBM）、中国期刊全文数据库（CNKI）、中文科技期刊全文数据库（VIP）,检索时间截至2013年11月。纳入研究为绿茶与胃癌发生风险相关性的队列研究。采用Cochrane协作网推荐的RevMan 5.2软件进行Meta分析。结果共纳入12个队列研究。未调整混杂因素的数据合并结果显示：与绿茶最低摄入量的暴露因素相比,绿茶最高摄入量的暴露因素人群在随访过程中胃癌的发生风险方面差异有统计学意义（RR=1.34,95%CI：1.10-1.65,P=0.005）;然而调整后的数据合并结果显示：与绿茶最小摄入量的暴露因素相比,绿茶最大摄入量的暴露因素人群在随访过程中胃癌的发生风险方面差异无明显的统计学意义（RR=1.02,95%CI：0.94-1.11,P=0.67）。亚组分析提示：女性每日绿茶最大摄入量暴露因素是胃癌发生的保护因素（RR=0.78,95%CI：0.64-0.95,P=0.01）。结论长期饮用绿茶可能会降低女性胃癌的发病风险,但并不能降低男性胃癌的发生风险。Objective To evaluate the relationship between green tea consumption and risk of gastric cancer. Methods Cochrane Library,PubMed,Embase,CBM,CNKI,and VIP were searched with relevant terms（to Nov. 2013）. Cohort study of green tea and gastric cancer risk was included. RevMan 5. 2 software was used for statistical analysis. Results Twelve cohort studies met inclusion criteria were analyzed. The results of meta analysis of crude data indicated that the gastric cancer risk of the population with the highest intake of green tea had statistically significant difference with that with the lowest intake of green tea（ RR = 1. 34,95% CI： 1. 10- 1. 65,P = 0. 005）. There was no statistical difference,however,when data was adjusted（RR =1.02,95%CI： 0.94-1.11,P =0.67）. The results from subgroup analysis indicated that the highest intake of green tea could reduce the risk of gastric cancer among female participants. Conclusion Long time consumption of green tea may reduce the risk of gastric cancer for female participants,but not for male participants.
