血清钙调蛋白与急性缺血性脑卒中的相关性研究

摘　　要：目的研究卒中后48小时钙调蛋白(CAM)水平与脑梗死面积的关系,探讨急性缺血性脑卒中(IS)患者CAM水平是否与急性期病情严重程度呈正相关。方法将研究对象分为对照组和实验组。正常对照组为本院100例健康体检者。实验组为入组的急性IS的120例患者,在入院时对IS患者依据头颅CT计算脑梗死面积,按照Adamas分类法,将120例IS患者再分为:大梗死组(27例)、小梗死组(42例)、腔隙性脑梗死组(51例)三组。依据分组情况测定血清CAM值,探讨IS后48小时CAM水平与脑梗死灶面积之间的关系,是否与急性期病情严重程度有相关性。结果实验组较对照组CAM值明显升高,有明显统计学意义(t=15.34,P<0.001)。大梗死组、小梗死组、腔隙性脑梗死组三组患者血清CAM值比较有显著差异(F=260.89,P<0.001)。结论急性缺血性卒中患者48小时CAM水平与脑梗死面积呈正相关,可作为一重要指标反映脑卒中患者急性期病情的严重程度。Objectives To investigate the relationship between the level of calmodulin obtained within 48 hours of symptom onset in patients after acute ischemic stroke with the infarction size and find whether the serum levels of calmodulin are positively correlated with acute illness severity.Methods The court＇s healthy subjects were as the healthy control group and the enrolled with acute ischemic stroke as the experimental group.The infarct area based on the cranial CT according to the classification of Adamas was calculated.The 120 cases of IS patients were subdivided into three groups：large infarction （27cases）,small infarction （42cases）,lacunar infarction （51cases）.The different period CAM value based on these groups,investigate the relationship between CAM levels of acute ischemic stroke patients with different infarct size were measured.Results The serum calmodulin level of IS group was significantly higher than that of the normal control group （t=15.34,P〈0.001）.The serum CAM levels of the three groups had significant difference（F=260.89,P〈0.01）.Conclusion The serum CAM level obtained within 48 hours of symptom onset in patients after acute ischemic stroke with the infarction size was positively correlated.It can be used as an important indicator of the acute phase of the disease in patients with stroke severity.
