奥氮平对精神分裂症患者认知功能和生活质量改善作用及疗效

摘　　要：目的研究奥氮平对精神分裂症患者认知功能及生活质量的改善作用及疗效。方法 60名受试者随机分为两组,分别给予奥氮平(n=30)和氟哌啶醇(n=30)治疗。分别在基线期、第8周、第16周和第24周进行阳性与阴性症状评定量表(PANSS)、生活质量与满意度量表评分及认知功能测查,并分析结果。结果奥氮平组持续性操作实验的反应时间在第24周(t=3.17,P<0.05),数字广度评分在第8周、第16周和第24周时均与基线有显著性差异(t=2.45,6.03,7.20;P<0.05);而氟哌啶醇组没有此差异。奥氮平组在数字广度评分上的表现优于氟哌啶醇组(F=5.10,P<0.05)。奥氮平组生活质量及满意度评分的改善与基线相比有显著性差异(F=3.32,P<0.05),而氟哌啶醇组的这种差异不显著。奥氮平组与氟哌啶醇组各时点PANSS量表分与基线相比均有显著性差异(F=17.96,P<0.001),但两组间差异不具显著性。结论奥氮平对精神分裂症患者的注意、工作记忆和生活质量有一定的改善作用,且对工作记忆的改善作用优于氟哌啶醇。两者在改善精神症状的临床疗效相当。Objective To investigate the efficacy of olanzapine in improving cognitive function ,quality of life and clinical symptom in schizophrenia.Methods Patients were randomized to either olanzapine （n=30）or haloperidol（n=30）treatment group.They were reexamined the clinical characters ,cognition abilities and quality of life at follow -up intervals that ranged up to 24 weeks.Results Olanzapine significantly improved the scores of continuous performance test （ CPT） at 24 week,and improved the scores of Digit Span Test at 8,16 and 24 week,compared with baseline（P〈0.05）,but not in haloperidol.Olanzapine performed better on Digit Span Test than haloperidol（P〈0.05）.Olanzapine significantly improved the scores of Quality of Life Enjoyment and Satisfaction Questionnaire （F=3.32,P〈0.05）,compared with baseline,but not in haloperidol.There were no inter group difference.Both olanzapine and haloperidol groups had significant improvement in PANSS scores （F=33.33 and 66.00 respectively,P〈0.001）,and there were no significant difference between them .Conclusion Olanzapine can improve the attention , working memory and quality of life in schizophrenia patients.Patients of long term treatment with olanzapine perform better on working memory than treatment with haloperidol .Both olanzapine and haloperidol show similar clinical effects on psychiatric symptom in schizophrenia .
