摘 要:目的探讨:大蒜素对细菌感染所致呼吸机相关性肺炎患者预后的影响。方法选择入住我科使用呼吸机的患者中筛选符合呼吸机相关性肺炎诊断标准的重痘患者(均为细菌感染所致)。随机分为治疗组(30例)及对照组(26例),对照组给予常规治疗,治疗组在常规治疗基础上加用高浓度大蒜素,分别在治疗后3天,7天,14天时比较两组患者APACHEⅡ评分,机械通气时间,住ICU时间及痰培养细菌转阴率之间是否有差别。结果两组患者入组时基本资料及APACHEⅡ评分没有差别。在治疗第3天和第7天,对照组与治疗组的患者APACHEⅡ评分均值没有差别,但在治疗第14天时,治疗组APACHEⅡ评分低于对照组,差异有显著性。治疗组患者机械通气时间短于对照组,差异有统计学意义。治疗组患者住ICU时间显著短于对照组,差异具有显著性。结论细菌感染所致的呼吸机相关性肺炎的患者中,在接受常规治疗的基础上加用高浓度大蒜素,能降低患者的APACHEⅡ评分,缩短机械通气时间及住ICU时间,并提高细菌转阴率,从而改善患者预后。Objective To investigate the effect of allicin in the prognosis of ventilator-associated pneumonia induced by bacterial infection. Methods Patients who were eligible to the diagnostic standard of ventilator-associated pneumonia (induced by bacterial infection) were selected from ventilator used patients in our department. They were randomly divided into treatment group (30 cases) and control group (26 cases). Patients in the control group received routine treatment, while patients in the treatment group were given high concentration of allicin on the basis of the routine treatment. Then the APACH Ⅱ score, mechanical ventilation time, the days stay in ICU and the negative conversion ratio of bacteria in sputum culture were compared after treatment of day 3, day 7 and dayl4.Results The basic data and APACH Ⅱ score showed no difference between two groups. At day 3 and day 7, there were no differences between two groups in the mean APACH Ⅱ score. But at day 14, the APACH Ⅱ score of the treatment group was significantly lower than that in the control group, with significant difference. The mechanical ventilation time in the treatment group was shorter than that in the control group, with statistical significant difference. The days stay in ICU in treatment group was significantly shorter than that in the control group, with significant difference.Conclusion In bacterial infection induced ventilator-associated pneumonia patients, treated patients with high concentration of allicin on the basis of routine treatment may reduce APACH Ⅱ score, shorten mechanical ventilation time and days stay in ICU, and enhance the negative conversion ratio of bacteria, therefore, the prognosis of patients may be improved.
