摘 要:目的:探讨结构化临床考试(OSCE)对临床医学生考试心理状况的影响。方法:选取笔者所在学校2008级临床医学专业300名学生作为研究对象,按照随机数字表法分为试验组和对照组各150名,对其制定相关医学临床能力考核方案,分别在预实习前、预实习结束OSCE与临床实习结束OSCE做心理健康症状自评量表SCL-90调查,研究不同民族、不同家庭背景的医学生OSCE对其心理所造成的影响。对照组采用传统考试方法,而试验组采用结构化临床考试。在考试前3个月及考试时对学生做心理健康症状自评量表SCL-90调查。观察比较两组学生在考试前后、不同家庭背景心理状态的差异。结果:两组学生考试时SCL-90评分总分、焦虑及抑郁评分均明显高于考试前3个月,且试验组学生考试时上述各评分均明显高于对照组,差异均有统计学差异(P<0.05)。农村学生考试前后的SCL-90评分均明显高于城镇学生,差异均有统计学意义(P<0.05)。结论:结构化临床考试可以客观、准确评估临床医学生临床能力,但其不同程度会造成考生焦虑、抑郁等心理问题。农村学生考试前后的心理问题问题明显高于城镇学生,在考试前应加强学生心理疏导教育。Objective:To explore the effect of structured clinical examination (OSCE) on psychological status examination of clinical medical students. Method:300 students of 2008 clinical medicine in our college were selected as the research object,they were randomly divided into the experimental group and the control group,150 cases in each group.The relevant clinical ability assessment scheme was formulated.The mental health symptom checklist SCL-90 for the students before the pre-practice,pre-internship OSCE and clinical internship was used for survey.The psychological effect of OSCE of different nationalities, different family backgrounds were discussed for survey.The control group was taken with traditional methods,and the experimental group was taken with structured clinical examination.The students were taken with mental health symptom checklist SCL-90 investigation three months before the exam and during exam.The differences of mental state and different family background before and after the test were compared.Result:The Symptom Checklist (SCL-90) total scores,anxiety and depression scores during the exam in the two group of students were significantly higher than those of before the exam 3 months,and the above scores during the exam in the experimental group were significantly higher than those in the control group,the differences were statistically significant(P〈0.05).The SCL-90 scores of rural students before and after examination were significantly higher than those of urban students,the differences were statistically significant(P〈0.05). Conclusion:The structured clinical examination can use to assess the medical students in clinical skills objectively.But it can cause varying degrees of anxiety, depression and other psychological problems.Psychological problems of rural students before and after test are significantly higher than urban students.The psychological counseling education should be strengthened before exam.
