摘  要:目的通过磁共振三维时间飞跃法(3D-TOF),探讨颅内动脉成窗变异磁共振血管造影(MRA)的发生率、好发部位及其临床意义。方法收集行颅颈部磁共振3D-TOF检查的患者资料1445例,将原始图像经工作站处理,得到最大密度投影(MIP)和容积重组(VR)图像,分析颅内动脉成窗变异的影像学特点。结果 1445例病例中,发现颅内动脉成窗变异38例(检出率2.63%,38/1445),其中位于基底动脉16例(检出率1.11%,16/1445);大脑前动脉12例(检出率0.83%,12/1445);椎动脉6例(检出率0.42%,6/1445);大脑后动脉3例(检出率0.21%,3/1445);大脑中动脉1例(检出率0.07%,1/14451)。38例成窗变异病例中有1例合并动脉瘤。结论磁共振3D-TOF能清楚显示和诊断颅内动脉成窗变异这一少见血管异常。Objective To explore the incidence, predilection site, and clinical significance of intracranial artery window-type variation magnetic resonance angiography(MRA), through magnetic resonance three dimensional time of flight(3D-TOF). Methods The data of 1445 patients, who underwent magnetic resonance 3D-TOF, were collected. The images of maximum intensity projection(MIP) and volume reformation(VR) were showed after processing of workstation in the original images. Imaging characteristics of intracranial artery window-type fenestration were analyzed. Results Among the 1445 cases, there were 38 cases with intracranial artery window-type variation(detection rate as 2.63%, 38/1445), and 16 of them in the basilar artery(detection rate as 1.11%, 16/1445), 12 cases in the anterior cerebral artery(detection rate as 0.83%, 12/1445), 6 cases in the vertebral artery(detection rate as 0.42%, 6/1445), 3 cases in the posterior cerebral artery(detection rate as 0.21%, 3/1445), and 1 case in the middle cerebral artery(detection rate as 0.07%, 1/1445). There was 1 of the 38 variation cases which combined with arterial aneurysm. Conclusion Intracranial artery window-type variation, as a rare vascular anomaly, can be clearly showed and diagnosed by magnetic resonance 3D-TOF.
