摘  要:目的探讨Fascin、MMP-7表达在大肠管状腺瘤癌变过程中的作用。方法采用免疫组织化学ElivisionTMplus两步法,检测40例大肠黏膜、82例大肠管状腺瘤、76例大肠管状腺瘤癌变中Fascin和MMP-7的表达情况。结果在大肠黏膜、大肠管状腺瘤、大肠管状腺瘤癌变中Fascin阳性表达率分别为5.0%、30.5%和64.5%,各组间差异均有统计学意义(均P<0.05);在大肠黏膜、大肠管状腺瘤、大肠管状腺瘤癌变中MMP-7阳性表达率分别为2.5%、32.9%和86.8%,各组间差异均有统计学意义(均P<0.05)。在大肠管状腺瘤癌变中Fascin、MMP-7阳性表达率均逐渐增高,且呈正相关(P<0.05)。Fascin、MMP-7表达与肿瘤的分化程度、浸润深度、淋巴结转移、Dukes分期有关(P<0.05);而MMP-7的表达还与年龄有关(P<0.05)。结论 Fascin、MMP-7共同参与了大肠管状腺瘤癌变的发生。Objective To investigate the expression of fascin and MMP-7 in sporadic colorectal tubular adenoma and colorectal tubular adenoma with cancerous changes and explore their role in the carcinogenesis of colorectal adenoma.Methods The expression of Fascin and MMP-7 was studied in 82 patients with sporadic colorectal tubular adenoma with different dysplasia,76 patients of colorectal tubular adenoma with cancerous changes and 40 patients of colorectal mucosa with ElivisionTMplus two-step.Results The positive expression rate of Fascin was 5.0%,30.5% and 64.5% and those of MMP-7 was 2.5%,32.9% and 86.8% respectively in colorectal mucosa,sporadic colorectal tubular adenoma with different dysplasia and colorectal tubular adenoma with cancerous changes.The positive correlation between Fascin and MMP-7 expression could be found in the carcinogenesis process of sporadic colorectal tubular adenoma.The expression of Fascin and MMP-7 was respectively related with the tumor differentiation,the depths of the tumor invasion,lymphatic metastasis and Ducks stages,the MMP-7 expression was related with the age.Conclusion Fascin and MMP-7 may be involved in carcinogenesis process of colorectal tubular adenoma.
