非小细胞肺癌不同Notch1表达水平的临床病理特征及预后

摘　　要：目的 探讨Notch1受体在非小细胞肺癌的发生、发展以及患者预后中所起的作用.方法 检索筛选出15篇高质量文献,运用Meta分析研究Notch1受体与非小细胞肺癌发生、发展及患者预后的关系.结果 非小细胞肺癌组织中Notch1表达水平是正常肺组织10.5倍（P＜0.01）,其表达水平与非小细胞肺癌（NSCLC）的病理类型及淋巴结转移无明显相关（P＞0.05）,与NSCLC的术后病理分期呈正相关,“Ⅲ＋Ⅳ期”患者的Notch1高表达比值为“Ⅰ＋Ⅱ期”的2倍（P＜0.01）;同高表达组比较,Notch1低表达组具有更高的5年生存率,并且降低了63％的死亡风险（P＜0.01）.结论 Notch1与非小细胞肺癌的发生、发展密切相关,其表达水平与术后病理分期呈正相关,高表达预示患者预后不良.Objective To collect all the related researches and pool the results quantitatively with the method of meta-analysis in order to assess the role of Notch1 receptor in the development of non-small cell lung cancer （NSCLC） and its relationship with prognosis.Methods Fifteen papers were collected for studies related to the expression of Notch receptor and survival analysis in NSCLC to discover the relationship between Notch1 receptor and the onset and development of NSCLC,respectively.Results The Notch1 expression level in NSCLC was significantly higher than that of normal lung tissue （P 〈 0.01）.No significant association between the expression level of Notch1 and subtypes of NSCLC was found in both East Asian subgroup （P 〉 0.05） and the whole study （P 〉 0.05）.There was no significant relationship between the Notch1 level and metastasis of lymph node （P 〉 0.05）.The Notch1 level was associated with pathological stage positively （P 〈0.01）.Lower expression of Notch1 increased 5-year survival rate significantly as compared with the higher Notch1 expression （P 〈 0.01）.Conclusion The expression of Notch1 is associated with the onset and development of NSCLC.The Notch1 expression level is positively correlated with pathological stage.In contrast,no significant association is found between Notch1 level and subtypes of NSCLC as well as metastasis of lymph node.Over-expression of Notch1 may result in poor prognosis.
