摘 要:目的探讨肺癌组织中脆性组氨酸三聚体(Fragile Histidine Triad,FHIT)和Raf激酶抑制蛋白(Raf Kinase Inhibitor Protein,RKIP)的表达及其与临床病理特征的关系。方法应用免疫组织化学方法检测132例肺癌组织及相应癌旁正常肺组织中FHIT与RKIP蛋白的表达情况,分析两者与肺癌临床病理学参数的关系。结果肺癌组织中FHIT的阳性表达率为31.82%(42/132),低于正常肺组织的73.48%(97/132),差异有统计学意义(χ2=45.963,P<0.05)。肺鳞癌中FHIT的阳性表达率低于肺腺癌,差异有统计学意义(P<0.05),吸烟组FHIT的阳性表达率低于非吸烟组,差异有统计学意义(P<0.05)。肺癌组织中RKIP的阳性表达率为33.33%(44/132),低于正常肺组织的75.76%(100/132),差异有统计学意义(χ2=47.911,P<0.05)。RKIP的阳性表达率与病理类型及吸烟史无关(P>0.05)。FHIT及RKIP的表达水平与肺癌的分化程度、p TNM分期、淋巴结转移及术后生存时间均有明显关系(P<0.05);FHIT蛋白与RKIP蛋白的表达呈正相关(P<0.05)。结论 FHIT与RKIP的缺失在肺癌的发生、发展中起重要作用,RKIP及FHIT的低表达促进了肺癌细胞的侵袭和转移,两者的联合检测为肺癌的诊断及预后判断提供了重要的临床价值。Objective The aim of this study was to investigate the expression of fragile histidine triad(FHIT) and raf kinase inhibitor protein(RKIP) in lung carcinoma, in order to explore its correlation with the clinicopathologic features. Methods The expressions of FHIT and RKIP were investigated using immunohistochemistry in 132 cases of lung cancer specimens and 132 cases of adjacent normal lung tissue samples. Statistical analysis was used to determine the relationship between the expressions and clinicopathological parameters. Results Statistical analysis revealed that the positive staining rate of FHIT was 31.82%(42/132) in tumor tissues, which was lower than that in normal lung tissues 73.74%(97/132)(χ^2=45.963, P〈0.05). The positive staining rate of FHIT in lung squamous carcinoma tissues was lower than that in lung adenocarcinoma(P〈0.05). The positive expression rate of FHIT in smoking group was lower than that in non-smoking group(P〈0.05). The positive staining rate of RKIP was 33.33%(44/132) in tumor tissues, which was lower than that in normal lung tissues 75.76%(100/132)(χ^2=47.911, P〈0.05). RKIP had nothing to do with the pathological type and smoking history. The expression of FHIT and RKIP were significantly related with the different differentiation of tumor, TNM staging, the development of lymph node metastasis and the survival time(P〈0.05,respectively). There was positive relationship between the expression of FHIT and RKIP in lung carcinoma(P〈0.05). Conclusion The loss of FHIT and RKIP may play an important role in the occurrence and development of lung cancer. The low expressions of FHIT and RKIP promote the invasion and metastasis of lung cancer. The combined detection of FHIT and RKIP has important clinical value in the diagnosis and prognosis of lung cancer.
