摘 要:目的探讨荧光原位杂交(FISH)技术诊断孕早期胚胎停育和自然流产绒毛染色体异常的临床应用价值。方法采用7种探针,应用FISH技术针对13、16、18、21、22号和X、Y染色体,对60例孕早期胚胎停育和自然流产绒毛组织进行检测分析,同时用30例人工流产绒毛作为对照。结果 60例孕早期胚胎停育和自然流产绒毛组织中32例存在染色体数目异常,检出率为53.33%,其中15例16-三体,5例13-三体,4例22-三体,1例21-三体,10例三倍体。30例人工流产绒毛组织中检出染色体数目异常1例,为16-三体,检出率为3.33%,孕早期胚胎停育和自然流产绒毛组织染色异常检出率明显高于人工流产绒毛组织染色体异常,差异有统计学意义(P<0.05)。结论染色体数目异常是导致孕早期胚胎停育和自然流产的主要因素,其中16号染色体三倍体是主要改变,FISH技术可以快速、简便地检测出流产绒毛组织染色体异常。Objective To investigate the application value of detection the chromosome abnormalities of chorionic villi tissue of early embryo damage and spontaneous abortion by fluorescent in situ hybridization technology.Methods Seven probes were used to detect 13,16,18,21,22 and X,Y respectively,and sixty cases of early embryo damage and spontaneous aborted fetuses chorionic villus were analyzed by FISH,and the results were compared with villi tissue from 30 cases of artificial abortion.Results Thirty two of the sixty cases were found chromosomal numerical aberrations,and the detection rate was 53.33%,including fifteen cases of 16-trisomy,five cases of 13-trisomy,four cases of 22-trisomy,one case of 21-trisomy and ten cases of triploidy.One of the thirty cases of artificial abortion were found chromosomal numerical aberrations,and the detection rate was 3.33%,which was much lower than that of early embryo damage and spontaneous abortion group.The difference has statistical significance(P〈0.05).Conclusion Chromosomal numerical aberrations are the major factors for early embryo damage and spontaneous abortion,and the major change occurred in 16-trisomy.FISH analysis is a rapid and easy method in the detection of chromosomal abnormality in chorionic villus.
